摘要 目的：研究加味补阳还五汤联合α-硫辛酸治疗气虚血瘀型糖尿病周围神经病变及尿NAG与DPN的相关性。方法：124例气虚血瘀型糖尿病周围神经病变患者随机分为对照组和观察组62例。对照组采用α-硫辛酸治疗，观察组在对照组治疗基础上加服加味补阳还五汤，连续治疗3个疗程，观察两组患者临床疗效及尿NAG变化情况。结果：观察组治疗总有效率91.93%明显高于对照组74.19%（P＜0.05）。治疗后两组中医症候积分明显降低，与治疗前对比（P＜0.05），且观察组降低明显优于对照组（P＜0.05）。治疗后两组神经传导速度明显改善，与治疗前对比（P＜0.05），且观察组改善明显优于对照组（P＜0.05）。治疗后两组尿NAG明显降低，与治疗前对比（P＜0.05），治疗后观察组与对照组对比（P＞0.05）。两组尿NAG明显高于无神经病变糖尿病患者和健康者（P＜0.05），无神经病变糖尿病患者与健康者对比（P＞0.05）。结论：加味补阳还五汤联合α-硫辛酸治疗气虚血瘀型糖尿病周围神经病变疗效确切，可改善患者临床症状，降低尿NAG，提高神经传导速度。
Abstract:
Abstract Objective：To study the relationship between the treatment of qi deficiency and blood stasis type diabetic peripheral neuropathy and urinary NAG and DPN by Jiawei Buyang Huanwu decoction combined with α-lipoic acid. Methods：124 cases of diabetic peripheral neuropathy with Qi-deficiency and blood-stasis syndrome were were randomly divided into control group and observation group with 62 patients in each group. The control group was treated with α-lipoic acid. The observation group was given Jiawei Buyang Huanwu decoction on the basis of the control group. The clinical efficacy and urinary NAG changes were compared between the two groups . Results ：The total effective rate of observation group was 91.93%，significantly higher than 74.19% of control group (P＜0.05). After treatment，the scores of TCM symptoms in the two groups were significantly lower than those before treatment (P＜0.05)，and the reduction in the observation group was significantly better than that in the control group (P＜0.05).2 groups of nerve conduction velocity was significantly improved after treatment，compared with before treatment (P＜0.05)，and the observation group improved significantly better than the control group (P＜0.05). The 2 groups after treatment of urinary NAG decreased significantly，compared with before treatment (P＜0.05)，the observation group after treatment compared with the control group (P＞0.05). The observation group and the control group of urine NAG was significantly higher than those without neuropathy in diabetes and healthy subjects (P＜0.05)，the patients without diabetic neuropathy and healthy comparison (P＞0.05). Conclusion ：The treatment of qi deficiency and blood stasis type diabetic peripheral neuropathy by Jiawei Buyang Huanwu decoction combined with α-lipoic acid is effective in improving clinical symptoms，reducing urinary NAG，increasing nerve conduction velocity and deserving clinical promotion.
