摘 要 目的:探讨小柴胡汤加减方治疗上呼吸道感染发热的治疗效果。方法:选择80例上呼吸道感染发热患者,按照随机数字表法分为观察组和对照组各40例,观察组给予小柴胡汤加减方治疗,对照组给予抗感染,退热治疗。观察比较两组的治疗总有效率,主要症状消失时间,血清反应蛋白(CRP)、β2微球蛋白(β2MG)、前白蛋白(PA)及血沉(ESR)、T淋巴细胞亚群、免疫球蛋白情况。结果:观察组总有效率为97.50%,明显高于对照组(75.00%),差异具有统计学意义(P<0.05);观察组发热反复次数(2.36±0.62)次,平均发热时间为(1.24±0.73) d,咽部充血消失时间(2.12±0.24) d,咳嗽消失时间(2.89±0.22) d,流涕停止时间(2.14±0.24) d,咳痰停止时间(2.65±0.59) d,均显著少于对照组各症状消失时间(P<0.05);治疗后观察组血清CRP、β2MG、ESR水平明显低于对照组,PA水平明显高于对照组(P<0.05);临床症状体征发热、咳嗽、咳痰、鼻塞、纳少等中医证候积分及总积分明显低于对照组(P<0.05);T淋巴细胞亚群CD4+、CD3+含量明显高于对照组,CD4+/ CD8+值明显大于对照组,IgG、IgM、IgA明显低于对照组,差异具有统计学意义(P<0.05)。结论:小柴胡汤加减方治疗上呼吸道感染发热疗效显著,可在短时间内降低患者体温,改善患者症状,提高患者免疫功能。 
Abstract:
Abstract Objective:To investigate the therapeutic effect of Xiaochaihu decoction in treating upper respiratory tract infection fever. Methods:The 80 patients with upper respiratory tract infections were selected and divided into observation group (n=40) and control group (n=40) according to the random number table method. The observation group was treated with Xiaochaihu decoction,the control group was given antiinfection,antipyretic therapy. the total effective rate of treatment,the disappearance time of main symptoms,serum reactive protein (CRP),β2microglobulin (β2MG),prealbumin (PA),ESR,and T lymphocyte subsets,immunoglobulin conditions were observed and compared.Results:The total effective rate in the observation group was 97.50%,which was significantly higher than that in the control group (75.00%),and the difference was statistically significant (P<0.05). The number of repeated episodes of fever (2.36±0.62) was observed in the observation group. The mean fever time was (1.24±0.73) days,the disappearance time of pharyngeal congestion (2.12±0.24)d,the time of disappearance of cough (2.89±0.22) days,and the time of flow cessation ( 2.14±0.24)d,cough stop time (2.65±0.59)d,both significantly less than the disappearance time of each symptom in the control group (P<0.05). After treatment,serum CRP,β2MG,and ESR levels in the observation group were significantly lower than those in the control group,the PA level was significantly higher than the control group (P<0.05).The TCM syndrome scores and total scores such as fever,cough,sputum,nasal congestion,and nausea were significantly lower than those in the control group (P<0.05). The content of CD4+ and CD3+ in T lymphocyte subsets was significantly higher than that in the control group,the CD4+/CD8+ values were significantly higher than those in the control group,IgG,IgM,and IgA were significantly lower than those in the control group,with a significant difference (P<0.05).Conclusion: Xiaochaihu decoction is effective in treating upper respiratory tract infections and can reduce the body temperature of patients in a short period of time,improve the symptoms of patients,and improve the immune function of patients. It has certain application value. 
