摘要:
摘 要 目的： 探讨括约肌切开术联合中药熏洗治疗Ⅱ、Ⅲ期肛裂的临床疗效及对疼痛和创面愈合情况的影响。方法：选择肛裂患者92例，采用随机数字表法分为对照组和观察组各46例。对照组采用括约肌切开术治疗，观察组采用括约肌切开术联合中药熏洗治疗。比较两组治疗后创面愈合时间、愈合瘢痕面积情况，记录并比较两组术后渗液、水肿、出血及疼痛积分变化情况，并对术后1周的临床疗效情况进行观察。结果：观察组治疗后创面愈合时间早于对照组，愈合后瘢痕面积少于对照组（P <0.05）；治疗后，观察组术后24、48、76 h的VAS评分积分、渗液积分、水肿积分、出血积分均明显比对照组低（P均＜0.05）。观察组总有效率100.00%（46/46）明显高于对照组84.78%（39/46），感染率6.52%（3/46）明显低于对照组21.74%（10/46）（P<0.05）。结论：括约肌切开术联合中药熏洗可明显减轻Ⅱ、Ⅲ期肛裂术后疼痛程度，促进创面愈合，降低术后感染率，临床疗效较好。
Abstract:
Abstract Objective: To investigate the clinical efficacy of sphincterotomy combined with fumigation and washing of Chinese herbs in the treatment of stage II and III anal fissure and its effect on pain and wound healing. Methods: Ninety-two patients with anal fissure admitted were randomly divided into control group and observation group, 46 cases each. The control group was treated with sphincterotomy, while the observation group was treated with sphincterotomy combined with fumigation and washing with traditional Chinese medicine. The wound healing time and scar area after treatment were compared between the two groups. The changes of osmotic fluid, edema, hemorrhage and pain scores were recorded and compared between the two groups. The clinical efficacy was observed one week after operation.Results: After treatment, the wound healing time of the observation group was earlier than that of the control group, and the scar area, the VAS scores, the osmotic scores , the edema scores of the observation group at 24, 48 and 76h after operation were significantly lower than those of the control group, and the bleeding scores of the observation group at 24, 48 and 76h after operation were significantly lower than those of the control group (P<0.05). The total effective rate of the observation group was 100.00% (46/46) significantly higher than that of the control group (84.78% (39/46), the infection rate was 6.52% (3/46) significantly lower than that of the control group (21.74% (10/46), the difference was statistically significant (P<0.05). Conclusion:Sphincterotomy combined with fumigation and washing with Chinese herbal medicine can obviously relieve the pain after operation of stage II and III anal fissure, promote wound healing and reduce the infection rate after operation. The clinical effect is remarkable and can be popularized and applied.
