摘 要 目的：探究穴位按摩配合穴位贴敷对经内镜下逆行胰胆管造影术（ERCP）诊断阻塞性黄疸患者外周血辅助性T细胞（Th17）/调节性T细胞（Treg）的平衡调节作用。方法：纳入ERCP诊治阻塞性黄疸患者104例，按照就诊顺序编号随机分为对照组和研究组各52例。对照组常规ERCP治疗，研究组围术期加用穴位按摩和穴位贴敷。观察两组入院时、出院时Th17/Treg、免疫功能、炎症指标、生活质量变化并比较。结果：出院时两组白细胞介素17（IL-17）、IL-23、Th17、Th17/Treg较入院时均显著下降，转化生长因子-β（TGF-β）、Treg则显著升高（P＜0.05），出院时研究组IL-17、IL-23、Th17、Th17/Treg显著低于于对照组，TGF-β、Treg显著高于对照组（P＜0.05）。出院时两组CD4+/CD8+、免疫球蛋白A（IgA）、IgG、IgM较入院时均显著升高（P＜0.05），研究组以上指标均显著高于对照组（P＜0.05）。出院时两组C反应蛋白（CRP）、前列腺素2（PGE2）、IL-6较入院时均显著下降（P＜0.05），出院时研究组以上指标均显著低于对照组（P＜0.05）。结论：穴位按摩配合穴位贴敷能改善ERCP血Th17/Treg平衡，提高免疫功能，抑制炎症反应。
Abstract:
Abstract Objective:To explore the effect of acupoint massage combined with acupoint application on the balance of helper T cells(Th17)/regulatory T cells(Treg) in peripheral blood of patients with obstructive jaundice diagnosed by endoscopic retrograde cholangiopancreatography(ERCP).Methods:104 patients with obstructive jaundice treated by ERCP were randomly divided into control group(52 cases) and research group(52 cases).The control group received routine ERCP treatment,while the study group received acupoint massage and acupoint application during perioperative period.The changes of Th17/Treg,immune function,inflammatory index and quality of life at admission and discharge were observed and compared between the two groups.Results:At discharge,the levels of IL-17,IL-23,Th17,Th17/Treg in the two groups were significantly lower than those in the control group,while the levels of TGF-beta and Treg were significantly higher than those in the control group(P＜0.05).At discharge,the levels of IL-17,IL-23,Th17,Th17/Treg in the study group were significantly lower than those in the control group,and the levels of TGF-beta and Treg were significantly higher than those in the control group(P＜0.05).At discharge,CD4+/CD8+,IgA,IgG and IgM in the two groups were significantly higher than those at admission(P＜0.05),and the above indexes in the study group were significantly higher than those in the control group(P＜0.05).CRP,PGE2 and IL-6 in the two groups were significantly lower than those in the control group at discharge(P＜0.05),and the above indexes in the study group were significantly lower than those in the control group(P＜0.05).Conclusion:Acupoint massage combined with acupoint application can improve blood Th17/Treg balance of ERCP,improve immune function,inhibit inflammatory reaction and improve quality of life.
