摘要:
摘要目的:探讨麻甘颗粒联合阿奇霉素治疗小儿支原体肺炎(MPP)的疗效及机制。方法:选取MPP 102例为研究对象,采用信封法随机分为观察组和对照组各51例,对照组给予阿奇霉素治疗,观察组在此基础上联合麻甘颗粒进行治疗,探讨对患者临床症状、炎症因子及免疫功能的影响。结果:观察组治疗有效率96.08%显著高于对照组80.39%(P<005);治疗后两组咳嗽气喘、痰黄而多、发热、咽红肿、舌红苔黄症状积分均显著降低,且观察组治疗后以上积分均明显低于对照组(P<005);治疗后两组肿瘤坏死因子(TNFα)、白介素6(IL6)、干扰素γ(IFNγ)、白介素10(IL10)、白介素4(IL4)水平均显著降低,白介素2(IL2)水平均显著升高,且观察组治疗后TNFα、IL6、IFNγ、IL10、IL4水平均明显低于对照组,IL2水平明显高于对照组(P<005);治疗后两组红细胞免疫复合物(RBCICR)、CD8+占比情况均显著降低,红细胞C3b受体(PBCC3bR)、CD3+、CD4+占比情况均显著升高,且观察组治疗后RBCICR、CD8+占比情况均明显低于对照组,PBCC3bR、CD3+、CD4+占比情况明显高于对照组(P<005);观察组不良反应发生率7.84%显著低于对照组25.49%(P<005)。结论:麻甘颗粒联合阿奇霉素治疗MPP疗效确切,可显著改善患者临床症状,另外患者的炎症因子、免疫功能均有所改变,推测麻甘颗粒联合阿奇霉素治疗MPP的机制可能与降低机体促炎因子水平,升高抗炎因子水平,改善机体免疫功能相关,且不良反应显著降低。
Abstract:
AbstractObjective:To investigate the curative effect and mechanism of Magan granules combined with azithromycin in the treatment of children with mycoplasma pneumoniae pneumonia (MPP).Methods:102 children with MPP were selected as subjects.They were randomly divided into observation group and control group by envekop method,51 cases in each group.The control group was treated with azithromycin while the observation group was treated with azithromycin combined with Magan granules.The curative effect was analyzed,and the effects on clinical symptoms,inflammatory factors and immune function were explored.Results:The response rate of the observation group was significantly higher than that of the control group (96.08% vs 80.39%,P<005).After treatment,scores of cough asthma,much yellow phlegm,fever,and red and swollen throat of both groups were significantly decreased.The scores of the observation group were significantly lower than those of the control group (P<005).After treatment,levels of tumor necrosis factor (TNFα),interleukin6 (IL6),interferonγ (IFNγ),interleukin10 (IL10) and interleukin4 (IL4) were significantly decreased,interleukin2 (IL2) levels were significantly increased.Levels of TNFα,IL6,IFNγ,IL10 and IL4 in the observation group after treatment were significantly lower than those in the control group,while the IL2 level was significantly higher than the control group (P<005).After treatment,ratios of red blood cell immune complex rosette (RBCICR) and CD8+ in both groups were significantly reduced,while ratios of red blood cell C3b receptor (PBCC3bR),CD3+ and CD4+ were significantly increased.Ratios of RBCICR and CD8+ in the observation group after treatment were significantly lower than those in the control group,while ratios of PBCC3bR,CD3+ and CD4+ were significantly higher than those in the control group〖LM〗 (P<005).The incidence of adverse reactions in the observation group was significantly lower than that in the control group (7.84% vs 25.49%,P<005).Conclusion:Magan granules combined with azithromycin is effective in the treatment of MPP.The combined treatment can significantly improve the clinical symptoms of patients.In addition,the inflammatory factors and immune function of patients are changed.It is speculated that the mechanism of Magan granules combined with azithromycin in the treatment of MPP may be related to reducing levels of proinflammatory factors,increasing levels of antiinflammatory factors,and improving the body's immune function. 
