摘要:
目的:观察自拟蠲痹化湿汤对寒湿型类风湿关节炎女性患者临床疗效、疾病活动度、血清指标的影响。方法:选取96例类风湿关节炎(RA)女性患者纳入本次研究,将入选患者按随机数字表法均分常规组与中药组,常规组给予西药治疗,中药在常规组的基础上加用本院自拟蠲痹化湿汤治疗。于1个疗程完成时,评估疗效; 同时比较两组中医证状积分,疾病活动度(DAS28)评分,生化指标:血沉(ESR)、类风湿因子(RF),血清指标:C反应蛋白(CRP)、肿瘤坏死因子α(TNF-α)、基质金属蛋白酶-3(MMP-3)、基质金属蛋白酶抑制剂-1(TIMP-1)于治疗前、后的异同。结果:总有效率为中药组95.83%,常规组83.33%,两组比较中药组更优,差异均有统计学意义(P<0.05); 治疗后,两组患者生化指标、DAS28评分、中医症状积分及血清指标均较本组治疗前改善,两组对比中药组更优,差异均有统计学意义(P<0.05)。结论:蠲痹化湿汤能够提高寒湿型类风湿关节炎女性患者的临床疗效,降低疾病活动度,全面改善各项血清指标与生化指标,对于提高患者预后质量具有积极意义。
Abstract:
Objective:To observe the effect of Juanbi Huashi decoction on clinical efficacy,disease activity and serum index of female patients with cold-dampness rheumatoid arthritis(RA).Methods:A total of 96 female patients with RA admitted were included in this study.The selected patients were divided into the conventional group and the traditional Chinese medicine(TCM)group according to the random number table method.After one course of treatment,the clinical efficacy of the two groups was compared.Further observation was focused on TCM syndrome integral,disease activity(DAS28)scores,biochemical indexes,blood sedimentation(ESR),rheumatoid factor(RF),serum index:c-reactive protein(CRP),tumor necrosis factor-α(TNF-α),matrix metalloproteinases-3(MMP-3),matrix metalloproteinases inhibitor-1(TIMP-1)expression level similarities and differences before and after treatment.Results:The total effective rate was 95.83% in the traditional Chinese medicine group and 83.33% in the conventional group.The two groups were better than the traditional Chinese medicine group(P<0.05),and the difference was statistically significant.After treatment,the biochemical indexes,DAS28 scores,TCM symptom scores and serum indexes of the two groups were improved compared with that of the group before treatment,and the two groups were better than the traditional Chinese medicine group(P<0.05),with statistically significant differences.Conclusion:Juanbi Huashi decoction can improve the clinical efficacy of female patients with cold-dampness RA,reduce the disease activity,comprehensively improve the serum indicators and biochemical indicators,and has positive significance for improving the prognosis quality of patients.
