摘要:
本文旨在探析曾普华教授基于“癌毒致虚”理论,治疗胰腺癌的辨治思路及临床用药特色。基于“癌毒致虚”理论及临床经验,曾师认为胰腺癌以“肝郁脾虚、毒瘀互结”为主,常兼夹湿热内蕴、水湿内停、胃失和降、气血亏虚、肝肾不足等证,当以“疏肝健脾、解毒化瘀”为基本治法,配合清热利湿、利水消肿、降逆和胃、补益气血、滋补肝肾等法。曾师认为胰腺癌早期治疗以疏肝健脾、解毒化瘀为主; 中期以扶正培本、解毒化瘀为主; 晚期以扶正培本为主,解毒化瘀为辅。基于“癌毒致虚”理论及临床经验,肝脾并治,多脏调燮,特色用药,在临床上已获得较好的疗效。
Abstract:
The purpose of this paper is to analyze Professor Zeng Puhua's thinking of the treatment and the characteristics of clinical medication of pancreatic cancer based on the theory of “cancer-toxicogenic deficiency”.Based on the theory and clinical experience of cancer-induced deficiency,Professor Zeng believes that pancreatic cancer is mainly characterized by “liver-stagnation and spleen-deficiency,combination of toxin and stagnation”,often accompanied with the syndromes of endoretention of damp-heat,internal stop of dampness,rise and fall misalignment of stomach Qi,deficiency of qi and blood and deficiency of liver and kidney.The basic treatment method is to “soothing liver and strengthening spleen,detoxifying and removing blood statis”,in combination with clearing heat-toxicity and eliminating dampness,inducing diuresis and alleviating edema,calming the adverse-rising energy and attempering stomach,tonifying Qi and blood,nourishing liver and kidney,etc.Professor Zeng deems that the early treatment of pancreatic cancer is mainly based on soothing liver and strengthening spleen,detoxifying and removing blood statis.In the middle stage,reinforcing the vital energy and consolidating the constitution,simultaneously detoxifying and removing blood statis.In the terminal stage,reinforcing the vital energy and consolidating the constitution is primary,and detoxifying and removing blood statis is secondary.Based on the theory and clinical experience of “cancer-toxicogenic deficiency”,the combined treatment of liver and spleen,multiple visceral adjustment and characteristic use of traditional Chinese medicine have obtained good clinical effect.
