摘要:
目的:探讨灌肠消炎方辅助西药治疗慢性盆腔炎疗效及对患者血液流变学、血清炎症因子水平的影响。方法:采用随机数字表法将122例慢性盆腔炎患者分为联合组与对照组各61例。对照组患者给予常规西药治疗,联合组患者在此基础上联合使用灌肠消炎方治疗。记录两组患者临床疗效,比较治疗前、治疗2周后两组患者中医证候积分(下腹痛、月经不调、白带异常、腰骶酸痛、疲软无力、行经腹痛加重)、血液流变学[全血高切黏度、全血低切黏度、红细胞沉降率(ESR)、纤维蛋白原(FIB)]及血清炎性因子[白介素-2(IL-2)、白介素-10(IL-10)、转化生长因子(TGF-β1)、单核细胞趋化因子(MCP-1)]水平的变化情况。结果:治疗2周后,联合组患者临床总有效率明显高于对照组(P<0.05); 两组患者中医证候积分均低于治疗前,且联合组明显低于同期对照组(P<0.05); 两组患者全血高切黏度、全血低切黏度、ESR、FIB水平较治疗前均明显降低,联合组低于同期对照组(P<0.05); 两组患者IL-2、IL-10水平均高于治疗前,且联合组高于对照组(P<0.05); 两组患者TGF-β1、MCP-1明显低于治疗前,且联合组明显低于同期对照组水平(P<0.05)。结论:灌肠消炎方辅助西药治疗慢性盆腔炎疗效显著,能有效减轻患者临床症状,改善血液流变学。
Abstract:
Objective:To explore the efficacy of TCM enema Xiaoyan prescription supplemented with western medicine in the treatment of chronic pelvic inflammatory disease and its influence on hemorheology and serum inflammatory factors.Methods:A total of 122 patients with chronic pelvic inflammatory disease were divided into combined group and control group according to the random number table method,with 61 cases in each group.Patients in control group were treated with conventional western medicine,and patients in combined group were combined with TCM enema Xiaoyan prescription on this basis.The clinical efficacy was recorded in the two groups,and the TCM syndromes scores(lower abdominal pain,irregular menstruation,abnormal leucorrhea,lumbosacral pain,fatigue,increased abdominal pain during menstruation),hemorheology [whole blood high-shear viscosity,whole blood low-shear viscosity,erythrocyte sedimentation rate(ESR),fibrinogen(FIB)] and serum inflammatory factors [Interleukin-2(IL-2),Interleukin-10(IL-10),transforming growth factor(TGF-β1),monocyte chemotactic factor(MCP-1)] were compared between the two groups before treatment and after 2 weeks of treatment.Results:After 2 weeks of treatment,the total clinical effective rate in combined group was significantly higher than that in control group(P<0.05).The TCM syndromes scores in the two groups were lower than those before treatment,and the scores in combined group were significantly lower than those in control group at the same period(P<0.05).The levels of whole blood high-shear viscosity,whole blood low-shear viscosity,ESR and FIB in the two groups were significantly lower than those before treatment,and the levels in combined group were lower than those in control group at the same time period(P<0.05).The levels of IL-2 and IL-10 in the two groups were higher than those before treatment,and the levels in combined group were higher than those in control group(P<0.05).The levels of TGF-β1 and MCP-1 in the two groups were significantly lower than those before treatment,and the levels in combined group were significantly lower than those in control group at the same time period(P<0.05).Conclusion:TCM enema Xiaoyan prescription supplemented with western medicine has a significant efficacy in the treatment of chronic pelvic inflammatory disease,and can effectively relieve the clinical symptoms,improve hemorheology and reduce the serum inflammatory factors.
