摘要:
目的:研究稳定期慢性阻塞性肺病(COPD)使用益气活血方疗效及其对肺功能、炎症因子的影响。方法:选择COPD稳定期患者200例,随机分为对照组和观察组各100例,对照组采用常规西药,观察组服用西药加益气活血方,均治疗3个月。观察治疗结束后临床疗效,对比治疗前后患者第1秒用力肺活量(FEV1)、最大肺活量(FVC)、FEV1/FVC及血气指标,记录治疗前后血清及痰诱导液白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平,统计治疗前后慢阻肺评估测试问卷(CAT)和呼吸困难指数(mMRC)评分。结果:观察组治疗后临床有效率为86.00%,高于对照组74.00%(P<0.05)。观察组治疗后FEV1、FVC、FEV1/FVC均高于对照组(P<0.05); 与对照组相比观察组血气指标改善较好(P<0.05); 两组治疗后血清IL-8、TNF-α水平组间比较,差异无统计学意义(P>0.05),观察组治疗后诱导痰上清液IL-8、TNF-α水平均低于对照组(P<0.05); 观察组治疗后CAT量表、mMRC量表评分低于对照组(P<0.05)。结论:益气活血方治疗COPD稳定期的疗效较好,可通过改善血气指标、肺部功能及痰液中炎症因子,进而改善药物治疗效果。
Abstract:
Objective:To study the therapeutic effect of Yiqi Huoxue recipe on pulmonary function and inflammatory factors in stable chronic obstructive pulmonary disease(COPD).Methods:200 patients with stable COPD,the control group was treated with routine western medicine for 3 months,and the observation group was treated with Yiqi Huoxue recipe for 3 months.The clinical effect was observed after treatment.The forced vital capacity(FVC),FEV1/FVC,arterial oxygen saturation(SaO2),arterial partial pressure of oxygen(PaO2)and arterial partial pressure of carbon dioxide(PaCO2)were compared before and after treatment.The levels of interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)in serum and sputum induction fluid were recorded before and after treatment,and the scores of COPD assessment test(CAT)and modified medical research council dyspnea scale(mMRC)were calculated before and after treatment.Results:The clinical effective rate of the observation group after treatment was 86.00%,which was higher than that of the control group(P<0.05).After the treatment,FEV1,FVC and FEV1/FVC(P<0.05),SaO2 and PaCO2 in the observation group were higher than those in the control group(P<0.05),and PaO2 in the observation group was lower than that in the control group after treatment(P< 0.05).There was no significant difference in the levels of serum IL-8 and TNF- α(P>0.05),but the levels of IL-8 and TNF-α in the supernatant of induced sputum in the observation group were lower than those in the control group after treatment(P<0.05),and the scores of CAT scale and mMRC scale in the observation group were lower than those in the control group after treatment(P<0.05).Conclusion:Yiqi Huoxue recipe is effective in the treatment of stable COPD,which can control the progression of the disease by improving the pulmonary function,blood gas index and inflammatory factors in sputum.
