摘要:
目的:分析脊柱推拿联合常规康复治疗对缺血性脑卒患者后手功能障碍恢复的影响。方法:将94例缺血性脑卒中手功能障碍患者随机分为常规组和推拿组各47例。常规组实施常规康复干预,推拿组在常规组基础上联合中医脊柱推拿干预治疗,评定两组治疗疗效。采用神经功能缺损评分(NIHSS)、手神经功能缺损指数评估神经受损状况,采用简式上肢运动功能评分(Fugl-Meyer)、Barthel指数评分分别评估上肢运动功能、日常生活能力。结果:推拿组总有效率为95.74%,显著高于常规组的82.98%(P<0.05)。两组治疗前NIHSS评分、手神经功能缺损评分比较差异无统计学意义(P>0.05),治疗后推拿组NIHSS评分、手神经功能缺损评分均低于常规组(P<0.05)。两组治疗前Fugl-Meyer评分、Barthel指数评分比较差异无统计学意义(P>0.05),治疗后推拿组两项评分均高于常规组(P<0.05)。结论:脊柱推拿联合常规康复治疗可更好地促进缺血性脑卒中后手功能障碍患者手功能恢复,改善手神经缺损,从而提高日常活动能力。
Abstract:
Objective:To analyze the effect of spinal massage combined with routine rehabilitation therapy on recovery from hand dysfunction after ischemic stroke.Methods:94 patients with hand dysfunction after ischemic stroke were randomized into routine group(n=47)and massage group(n=47).The routine group was given routine rehabilitation intervention,and the massage group was treated with traditional Chinese medicine spinal massage based on routine intervention.Therapeutic effects of the two groups were evaluated.The national institutes of health stroke scale(NIHSS)and hand neurological impairment index were used to assess nerve damage.The Fugl-meyer motor assessment(Fugl-Meyer)score and barthel index score were used to evaluate upper limb motor function and activities of daily living,respectively.Results:The total effective rate of the massage group(95.74%)was significantly higher than that of the routine group(82.98%).There was no statistically significant difference in NIHSS score,hand neurological impairment score,Fugl-meyer score or barthel index score between the two groups before treatment(P>0.05).After treatment,NIHSS score and hand neurological impairment score of the massage group were significantly lower than those of the routine group(P<0.05),while Fugl-meyer score and barthel index score were significantly higher than those of the routine group.Conclusion:Spinal massage combined with routine rehabilitation therapy can better promote the recovery of hand function in patients with hand dysfunction after ischemic stroke and relieve hand neurological impairment,thus improving activities of daily living.
