摘要:
目的:观察健脾养胃汤加减治疗慢性胃炎的临床疗效,探讨炒白术的健脾益胃作用机制。方法:将203例慢性胃炎患者随机分为两组,对照组(n=101)以奥美拉唑(或雷尼替丁)、甲硝唑、阿莫西林和胶态铋制剂三联或四联用药,研究组(n=102)采用口服以炒白术为主的健脾养胃汤加减治疗,比较两组临床疗效及消化功能改善情况。结果:研究组总有效率显著高于对照组(97.06%与78.22%)。研究组患者的胃肠症状、全身症状、情感能力等生活质量评分和外周血中抗炎因子TGF-β水平显著高于对照组,促炎因子IL-1β水平则显著低于对照组,差异均有统计学意义(P<0.05)。结论:以炒白术为主的健脾养胃汤具有独特的健脾益胃功效,可抑制胃酸分泌,降低胃蛋白酶活性,减轻胃黏膜损伤。
Abstract:
Objective:To observe the clinical efficacy of Jianpi Yangwei decoction in treating chronic gastritis,and to explore the mechanism of Jianpi Yangwei decoction for invigorating the spleen and stomach.Methods:203 patients with chronic gastritis were randomly divided into two groups.The control group(n=101)was treated with omeprazole(or ranitidine),metronidazole,amoxicillin and colloidal bismuth preparations for medication,the study group(n=102)used oral treatment with the addition and subtraction of Jianpi Yangwei decoction,which mainly consisted of fried Atractylodes macrocephala,to compare the clinical efficacy and improvement of digestive function between the two groups.Results:The total effective rate of the study group was significantly higher than that of the control group(97.06% vs 78.22%).The quality of life scores such as gastrointestinal symptoms,systemic symptoms,and emotional ability of the patients in the study group and the expression level of anti-inflammatory factor TGF-β in peripheral blood were significantly higher than those in the control group,while the pro-inflammatory factor IL-1β level was significantly lower than that of the control group.The differences were statistically significant(P<0.05).Conclusion:The Jianpi Yangwei decoction based on fried Atractylodes macrocephala has unique spleen and stomach-invigorating effects,which can greatly inhibit gastric acid secretion,reduce pepsin activity and reduce gastric mucosal damage.
