摘要:
目的:探讨清肠解毒汤治疗溃疡性结肠炎湿热内蕴证以及对患者辅助性T细胞1/辅助性T细胞2(Th1/Th2)平衡的影响。方法:选取96例湿热内蕴证溃疡性结肠炎患者为研究对象,按随机数字表法,分为两组。对照组采用常规西药治疗,观察组增加清肠解毒汤治疗,两组均治疗3个月。对比两组湿热内蕴证中医症状评分,肠镜下黏膜表现评分和黏膜组织学Geboes指数评分,临床疗效,血清白细胞介素-4(IL-4)、干扰素-γ(IFN-γ)水平和IFN-γ/IL-4以及治疗期间不良反应。结果:治疗后两组患者湿热内蕴证中医症状评分均降低(P<0.05),且观察组更低(P<0.05); 治疗后两组肠镜下黏膜表现评分和黏膜组织学Geboes指数评分均降低(P<0.05),且观察组更低(P<0.05); 治疗后两组血清IL-4水平均升高(P<0.05),且观察组更高(P<0.05),两组血清IFN-γ水平及IFN-γ/IL-4均降低(P<0.05),且观察组更低(P<0.05); 两组不良反应发生率比较,差异无统计学意义(P>0.05); 治疗后观察组的总有效率高于对照组(P<0.05)。结论:清肠解毒汤用于治疗溃疡性结肠炎(湿热内蕴证)患者,能够改善临床症状,疗效确切,同时可调节Th1/Th2平衡。
Abstract:
Objective:To explore the effect of Qingchang Jiedu decoction in treating ulcerative colitis with internal accumulation of damp-heat and its effect on the balance of helper T cell 1/helper T cell 2(Th1/Th2).Methods:Ninety-six patients with ulcerative colitis with internal damp-heat syndrome were selected as the research subjects and divided into two groups according to the random number table.The control group was treated with conventional western medicine,and the observation group was treated with Qingchang Jiedu decoction,two groups were treated for 3 months.The traditional Chinese medicine symptom scores,mucosal performance scores under colonoscopy,geboes index scores of mucosal histology,clinical efficacy,serum interleukin-4(IL-4),interferon-γ(IFN-γ),IFN-γ/IL-4,and adverse reactions during treatment were compared.Results:After treatment,the traditional Chinese medicine symptom scores of the two groups of patients with damp-heat internal accumulation syndrome were decreased(P<0.05),and the observation group were lower(P<0.05).After treatment,the scores of mucosal performance and geboes index of mucosal histology in the two groups were decreased(P<0.05),and the observation group were lower(P<0.05).The serum IL-4 levels of the two groups were increased after treatment(P<0.05),and the observation group was higher(P<0.05),and the serum IFN-γ levels and IFN-γ/IL-4 of the two groups were decreased(P<0.05),and the observation group were lower(P<0.05).There were no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Compared with the control group,the total effective rate of observation group was higher(P<0.05).Conclusion:Qingchang Jiedu decoction can improve the clinical symptoms of patients with ulcerative colitis(damp heat syndrome)with definite curative effect,and adjust the balance of Th1/Th2.
