摘要:
目的:基于数据挖掘方法探究国医大师夏桂成教授治疗多囊卵巢综合征(PCOS)致不孕的学术特色,形成指导临床运用的实效方法。 方法:基于夏教授诊治的“不孕症”门诊病历数据库,建立“不孕因素:PCOS-证候-处方”主题模型,运用点式互信息、主题模型分析方法进行数据挖掘。 结果:不孕因素PCOS主要分布在主题4和5:主题4中四诊信息概率从高到低依次为带下量少、便秘、带下量极少等,处方概率从高到低依次为滋肾生肝饮合钩藤汤(丹参、荆芥、赤芍、钩藤、莲子心等)、清心滋肾汤(生龙齿、黄连、钩藤、炙龟板、莲子心等)和补天种玉汤合越鞠丸(香附、五灵脂、丹参、杜仲、赤芍)等; 主题5中四诊信息概率从高到低依次为便溏、大便先干后稀、月经量少等,处方概率从高到低依次为参苓白术散(砂仁、炒白术、木香、陈皮、建莲肉等)、补天种玉汤合香砂六君子汤(砂仁、木香、杜仲、炒白术、菟丝子等)和滋肾生肝饮、钩藤汤合香砂六君子汤(砂仁、荆芥、炒白术、赤芍、丹参等)。结论:“肾阴虚、心火旺,兼有脾胃失和证”和“脾胃失和,兼心肝火旺证”为PCOS致不孕的主要证型,“治疗重在经后期,因势利导、燮理阴阳,燮理心肾肝脾、促进卵泡发育”为夏教授治疗特色。
Abstract:
Objective:To explore the academic characteristics of infertility caused by polycystic ovary syndrome(PCOS)treated by Chinese medicine master professor XIA Guicheng based on data mining method and to form effective methods to guide clinical application.Methods:The topic model of infertility factors:PCOS-syndrome-prescription was established based on the medical record database of “infertility” outpatients treated by master XIA,and data mining was conducted by the point mutual information and topic model analysis.Results:Infertility factors PCOS were mainly distributed in themes 4 and 5.In topic 4,the probability of the four diagnostic methods information from high to low was low amount of leucorrhea,constipation,and little amount of leucorrhea,etc.The probability of prescription from high to low was Zishen Shenggan decoction combined with Gouteng decoction,Qinxin Zishen decoction and Butian Zhongyu decoction combined with Yueju pill. In topic 5,the probability of the four diagnostic methods information from high to low was loose stools,dry stools first and then thin stools,and oligomenorrhea,etc.The probability of prescription from high to low was Shenling Baizhu decoction,Butian Zhongyu decoction combined with Xiangsha Liujunzi decoction,Zishen Shenggan decoction,Gouteng decoction combined with Xiangsha Liujunzi decoction.Conclusion:“Shen-Yin deficiency and Xin-fire hyperactivity,combined with Pi and Wei disharmony syndrome” and “Pi and Wei disharmony,combined with Xin and Gan-fire hyperactivity syndrome” were the main syndromes of infertility caused by PCOS.“The treatment focuses on follicular phase,capitalizing on the trend,regulating Yin and Yang,regulating Xin,Shen,Gan and Pi,promoting follicle development” are the characteristics of master XIA's treatment.
