摘要:
目的:探讨十全大补汤联合五味消毒散加减对剖宫产术后产妇感染、炎症因子及T淋巴细胞亚群的影响。方法:选取150例剖宫产产妇为研究对象,随机分为对照组和治疗组,每组各75例。对照组术后予以常规营养支持,腹部切口按时换药。治疗组术后排气后口服十全大补汤合五味消毒散加减,并于腹部切口外敷中药包。比较两组术后感染发生率、炎症因子[C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)、白细胞介素-10(IL-10)]水平、T淋巴细胞亚群及产后康复情况。结果:治疗组术后感染发生率低于对照组(P<0.05)。治疗后,治疗组血清CRP、TNF-α、IL-8、CD8+水平显著低于对照组(均P<0.05),IL-10、CD3+、CD4+、CD4+/CD8+水平高于对照组(均P<0.05)。治疗组切口愈合时间、术后住院时间及并发症发生率低于对照组(均P<0.05)。结论:剖宫产术后产妇应用十全大补汤联合五味消毒散加减有助于预防术后感染,减轻机体炎症反应,调节免疫,促进患者术后恢复。
Abstract:
Objective:To investigate the effects of modified Shiquan Dabu decoction combined with Wuwei Xiaodu power on postoperative infection,inflammatory factors and T lymphocyte subsets in cesarean section parturients.Methods:150 cases of cesarean section were selected and randomly divided into control group and treatment group,with 75 cases in each group.The control group received routine nutritional support after operation,and the abdominal incision was changed on time.The treatment group was treated with modified Shiquan Dabu decoction combined with Wuwei Xiaodu power,the abdominal incision was externally applied with Chinese medicine package.The incidence of postoperative infection,levels of inflammatory factors [C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-8(IL-8),interleukin-10(IL-10)],T lymphocyte subsets and postpartum rehabilitation were compared between the two groups.Results:The postoperative infection rate in the treatment group was significantly lower than that in the control group(P<0.05).After treatment,the levels of serum CRP,TNF-α,IL-8 and CD8+ in the treatment group were significantly lower than those in the control group(all P<0.05),and IL-10,CD3+,CD4+ and CD4+/CD8+ were significantly higher than those in the control group(all P<0.05).The wound healing time,postoperative hospital stay and incidence of complications in the treatment group were less than those in the control group(all P<0.05).Conclusion:Application of Shiquan Dabu decoction and Wuwei Xiaodu power after cesarean section can help prevent postoperative infection,reduce inflammation,regulate immunity and promote postoperative recovery of patients.
