摘要:
目的:研究解毒止血汤联合腕关节镜治疗对尺骨撞击综合征(UIS)患者腕关节功能的影响。方法:选取32例UIS患者,按照随机数字表法分为研究组、对照组,每组16例。研究组采取腕关节镜下尺骨短缩截骨内固定术,对照组采用腕关节镜下尺骨薄层切除术(Wafer术)治疗; 两组术后均口服解毒止血汤。随访半年,比较两组手术指标、疼痛视觉模拟评分(VAS)、腕关节Cooney评分、腕关节活动度,记录两组术后并发症发生情况。结果:研究组手术时间短于对照组,切口长度、术中出血量均大于对照组,差异有统计学意义(均P<0.05)。术后2周、术后1个月、术后2个月,研究组VAS评分均低于对照组,Cooney评分、腕关节活动度均高于对照组,差异有统计学意义(均P<0.05)。术后3、6个月,两组VAS评分、Cooney评分、腕关节活动度比较,差异无统计学意义(均P>0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:解毒止血汤联合两种腕关节镜术式治疗UIS疗效较好,其中尺骨短缩截骨内固定术近期效果更好,可有效减轻患者腕部疼痛,改善腕关节功能及活动度,远期效果与Wafer术相当,但Wafer术切口小、出血少。
Abstract:
Objective:To study effect of Jiedu Zhixue decoction combined with wrist arthroscopy on wrist function in patients with ulnar impaction syndrome(UIS).Methods:32 patients with UIS were divided into study group and control group according to random number table method,16 cases in each group.The study group treated with wrist arthroscopic internal fixation of ulnar shortening osteotomy.The control group treated with wrist arthroscopic thin-slice resection of the ulna(Wafer).After surgery,patients in both groups were given Jiedu Zhixue decoction.After half year of follow-up,surgical indexes,scores of visual analogue scale(VAS),wrist Cooney score,wrist range of motion in the two groups were compared.The occurrence of postoperative complications in both groups were recorded.Results:The operation time in the study group was shorter than that in the control group,incision length and intraoperative blood loss were greater than those in the control group,difference statistically significant(all P<0.05).At two weeks,one month and two months after surgery,VAS scores in the study group were lower than those in the control group,while Cooney scores and wrist range of motion were higher than those in the control group,difference statistically significant(all P<0.05).At three and six months after surgery,there was no significant difference in VAS scores,Cooney scores and wrist range of motion between the two groups(all P>0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Jiedu Zhixue decoction combined with two kinds of wrist arthroscopy has better effect on UIS.The short-term effect of ulnar shortening osteotomy for internal fixation is more significant,which can effectively alleviate wrist pain,improve wrist function and range of motion.Its long-term effect is comparable to that of Wafer.Incision of Wafer is smaller and blood loss less.
