摘要:
目的:探讨补肾活血安胎方治疗妊娠早期绒毛膜下血肿(SCH)疗效及对患者蛋白s(Ps)、蛋白c(Pc)、D-二聚体(D-D)和性激素水平的影响。方法:选取120例SCH孕妇分为中药组(n=37)、西药组(n=41)、中西药结合组(n=42),中药组予补肾活血安胎方治疗,西药组予地屈孕酮治疗,中西药结合组予补肾活血安胎方联合地屈孕酮治疗。比较三组血浆Ps、Pc、凝血因子Ⅷ、NK细胞、D-D及血清催乳素(PRL)、雌二醇(E2)、黄体生成素(LH)、人绒毛膜促性腺激素(β-HCG)、促卵泡激素(FSH)、孕酮(P)水平。比较三组孕妇妊娠结局和产科并发症发生情况。结果:中西药结合组在治疗后血浆凝血因子Ⅷ、NK细胞、D-D水平显著低于西药组、中药组,Ps、Pc活性显著高于西药组、中药组(均P<0.05); E2、P水平高于西药组、中药组,β-HCG、PRL水平显著低于西药组、中药组(均P<0.05); 中西药结合组总不良妊娠结局率、总产科并发症发生率低于西药组、中药组(均P<0.05)。结论:补肾活血安胎方可改善SCH患者Ps、Pc、D-D及血清孕激素水平,降低不良妊娠结局和产科并发症发生率。
Abstract:
Objective:To investigate the effect of Bushen Huoxue Antai prescription on the levels of protein S(Ps),protein C(Pc),D-dimer(D-D)and sex hormone in the treatment of subchorionic hematoma(SCH)in early pregnancy.Methods:120 SCH pregnant women were divided into traditional Chinese medicine group(n=37),western medicine group(n=41)and combination of Chinese and western medicine group(n=42).The traditional Chinese medicine group was treated with Bushen Huoxue Antai prescription,the western medicine group was treated with dydrogesterone,and the combination of Chinese and western medicine group was treated with Bushen Huoxue Antai prescription combined with dydrogesterone.The plasma levels of Ps,Pc,plasma coagulation factorⅧ,NK cells,D-D and the level of serum prolactin(PRL),estradiol(E2),luteinizing hormone(LH),human chorionic gonadotropin(β-HCG),follicle-stimulating hormone(FSH)and progesterone(P)in three groups were compared.The pregnancy outcomes and obstetric complications in three groups were observed and recorded.Results:After treatment,the plasma coagulation factorⅧ,NK cells and D-D levels in the combination group were significantly lower than those in the western medicine group and Chinese medicine group,and the activities of Ps and Pc were significantly higher than those in the western medicine group and Chinese medicine group(all P<0.05).After treatment,E2 and P levels in the combination group were higher than those in the western medicine group and Chinese medicine group,while β-HCG and PRL levels were significantly lower than those in the western medicine group and Chinese medicine group(all P<0.05).The total rate of adverse pregnancy outcome and incidence of obstetric complications in the combined group were lower than those in the western medicine group and Chinese medicine group(all P<0.05).Conclusion:Bushen Huoxue Antai prescription can improve Ps,Pc,D-D and serum progesterone levels in SCH patients,and reduce the incidence of adverse pregnancy outcomes and obstetric complications.
