摘要:
目的:观察严氏温阳化瘀利水方治疗慢性心力衰竭疗效及对患者血浆miR-133a水平的影响。方法:以慢性心力衰竭患者95例作为研究对象,应用随机数字表法将患者分为治疗组与对照组,其中治疗组47例,对照组48例。对照组采用西医基础治疗,治疗组在对照组治疗基础上服用温阳化瘀利水方。比较两组左心室射血分数、B型脑钠肽(BNP)、miR-133a、转化生长因子(TGF-β1)、心功能分级、中医证候疗效、明尼苏达心力衰竭生活质量调查表评分及安全性指标。结果:治疗后,两组射血分数均升高,且治疗组高于对照组(均P<0.05)。两组BNP、TGF-β1、miR-133a水平均降低,且治疗组低于对照组(均P<0.05)。治疗后,两组明尼苏达心力衰竭生活质量调查表评分降低,且治疗组低于对照组(均P<0.05)。治疗后,两组心功能分级与中医证候积分均改善,且治疗组改善程度优于对照组(均P<0.05)。随访1年,治疗组患者病死率低于对照组(P<0.05),两组再入院率比较差异无统计学意义(P>0.05)。结论:严氏温阳化瘀利水法联合西医治疗心力衰竭效果较好,可降低血清TGF-β1水平,抑制心肌纤维化及心室重构,改善心功能,降低血清miR-133a水平。
Abstract:
Objective:To observe the curative effect of Yanshi Wenyang Huayu Lishui decoction on chronic heart failure and its influence on plasma miR-133a level.Methods:A total of 95 patients with chronic heart failure were were divided into treatment group and control group by random number table method,including 47 patients in the treatment group and 48 patients in the control group.The control group was treated with western medicine,and the treatment group was treated with Yanshi Wenyang Huayu Lishui decoction on the basis of the treatment of the control group.Left ventricular ejection fraction,B-type brain natriuretic peptide(BNP),miR-133a,transforming growth factor(TGF-β1),cardiac function grading,TCM syndrome efficacy,Minnesota Heart Failure Quality of Life Questionnaire Score and safety indexes were compared between the two groups.Results:After treatment,the ejection fraction of both groups increased,and the treatment group was higher than that in the control group(all P<0.05).The levels of BNP,TGF-β1 and miR-133a in both groups were decreased,and the treatment group was lower than those in the control group(all P<0.05).After treatment,scores on the Minnesota Heart Failure Quality of Life Questionnaire were lower in the treatment group than that in the control group(all P<0.05).After treatment,cardiac function grading and TCM syndrome score were improved in two groups,and the improvement degree in treatment group was better than that in control group(all P<0.05).The mortality in the treatment group was lower than that in the control group(P<0.05),there was no significant difference in the readmission rate between the two groups(P>0.05).Conclusion:Yanshi Wenyang Huayu Lishui decoction combined with western medicine is effective in the treatment of heart failure,which can reduce the level of serum TGF-β1,inhibit myocardial fibrosis and ventricular remodeling,improve cardiac function,and reduce the level of serum miR-133a.
