摘要:
目的:观察促愈熏洗方对肛瘘术后创面愈合的临床疗效及创面肉芽组织中共刺激分子(CD86)、甘露糖受体1(CD206)、白介素-1β(IL-1β)表达的影响。方法:将93例肛瘘术后患者按随机数字表法分对照组(n=47)、观察组(n=46); 对照组接受痔疾洗液坐浴熏洗治疗,观察组接受促愈熏洗方坐浴熏洗治疗; 比较两组疗效、创面愈合率,记录创面疼痛、创缘水肿、创面分泌物情况,并检测两组创面肉芽组织中CD86、CD206、IL-1β表达及血清干扰素-γ(IFN-γ)、诱导型一氧化氮合酶(iNOS)、转化生长因子-β(TGF-β)水平。结果:两组疗效等级比较差异有统计学意义(P<0.05); 术后14、21、28 d观察组创面愈合率高于对照组(P<0.05); 与术后1 d比较,两组术后3、7、14 d的创面疼痛评分、创缘水肿评分、创面分泌物评分显著下降,观察组低于对照组(P<0.05); 观察组肉芽组织中CD86、IL-1β表达较对照组显著减弱,肉芽组织中CD206表达显著增强; 术后14 d,两组IFN-γ、iNOS水平显著下降、TGF-β水平均上升,观察组术后14 d IFN-γ、iNOS水平显著低于对照组,TGF-β水平高于对照组(P<0.05)。结论:促愈熏洗方用于肛瘘术后创面可提高疗效,改善创面局部免疫状态,抑制炎症反应,更利于创面愈合。
Abstract:
Objective:To observe the curative effect of Cuyu Xunxi formula on wound after anal fistula operation,and the influence on CD86,CD206 and IL-1β expression in wound granulation tissue.Methods:A total of 93 patients who had undergone anal fistula operation were divided into control group(n=47)and observation group(n=46)by random number table method.All patients were given conventional treatment.On this basis,patients in the observation group were treated with Cuyu Xunxi formula.Curative effect and wound healing rate were compared between the two groups.Wound pain,wound edge edema,and wound secretion were recorded.The expression of CD86,CD206 and IL-1β,IFN-γ,iNOS and TGF-β levels in wound granulation tissues of the two groups were detected.Results:There was a statistically significant difference in the grade of curative effect between the two groups.The recovery rate in the observation group was significantly higher than that in the control group(P<0.05).The wound healing rate in the observation group was significantly higher than that in the control group on day 14,day 21 and day 28 after operation(P<0.05).The wound pain scores,wound edge edema scores and wound secretion scores of the two groups were significantly lower on day 3,day 7 and day 14 after operation than on day 1 after operation.The observation group were significantly lower than those in the control group(P<0.05).The expression levels of CD86 and IL-1β were significantly lower,and the expression of CD206 was significantly higher in the observation group than that in the control group.The levels of IFN-γ and iNOS were significantly decreased,and the level of TGF-β was significantly increased in the two groups on day 14 after operation.The levels of IFN-γ and iNOS were significantly lower,and the level of TGF-β was significantly higher in the observation group than those in the control group on day 14 after operation(P<0.05).Conclusion:Cuyu Xunxi formula can improve the curative effect on wound after anal fistula operation,improve local immune state of the wound,and inhibit inflammatory response,thereby promoting wound healing.
