摘要:
目的:探讨补肺健脾方联合茚达特罗格隆溴铵对老年慢性阻塞性肺疾病(COPD)稳定期患者疗效及对磷脂酰肌醇激酶3/蛋白激酶B(PI3K/Akt)信号通路和炎症因子的影响。方法:选择老年COPD稳定期患者122例,运用随机数字表法分为两组,各61例。对照组给予茚达特罗格隆溴铵治疗,观察组给予补肺健脾方联合茚达特罗格隆溴铵治疗,两组疗程均为8周。比较两组治疗疗效,治疗前后肺功能、COPD评估测试(CAT)问卷评分、6 min步行距离(6MWT)、血清PI3K、血清Akt水平、PI3K/Akt蛋白表达,肿瘤坏死因子-α(TNF-α)、白介素-8(IL-8)和白介素-15(IL-15)水平。结果:观察组治疗总有效率高于对照组。治疗后,两组FEV1、PEF、FEV1/FVC和6MWT高于治疗前,而CAT评分、血清PI3K、血清Akt水平、PI3K/Akt蛋白表达灰度值、TNF-α、IL-8、IL-15水平低于治疗前,差异有统计学意义(均P<0.05)。治疗后,两组FEV1、PEF、FEV1/FVC和6MWT高于对照组,而CAT评分、血清PI3K、血清Akt水平、PI3K/Akt蛋白表达灰度值、TNF-α、IL-8、IL-15水平低于对照组,差异有统计学意义(均P<0.05)。结论:补肺健脾方联合茚达特罗格隆溴铵对老年COPD稳定期患者疗效较好,患者肺功能明显改善,生活质量、运动耐力明显提高,且PI3K/Akt信号通路表达下调,细胞炎症反应降低。
Abstract:
Objective:To investigate therapeutic effect of Bufei Jianpi recipe combined with indatrioglom ammonium bromide on elderly patients with chronic obstructive pulmonary disease(COPD)at stable stage and its effect on phosphatidylinositol kinase 3/protein kinase B(PI3K/Akt)signal pathway and cytokines.Methods:122 elderly patients with stable COPD were divided into two groups each 61 cases.The control group treated indatrogolone and ammonium bromide,and the observation group treated Bufei Jianpi recipe combined with indatrogolone and ammonium bromide.The two groups treatment course 8 weeks.The therapeutic effects of the two groups were compared.Pulmonary function,COPD assessment test(CAT)questionnaire score,6-minute walking distance(6MWT),changes in serum PI3K and Akt levels,PI3K/Akt protein expression,tumor necrosis factor-α(TNF-α),interleukin-8(IL-8)and interleukin-15(IL-15)were compared before and after treatment.Results:The observation group effective was higher than that in the control group.After treatment,two groups FEV1,PEF,FEV1/FVC and 6MWT were higher than before treatment,while CAT score,serum PI3K and Akt levels,gray value of PI3K and Akt protein expression,serum TNF-α,IL-8 and IL-15 levels lower before treatment,difference statistically significant(all P<0.05).The observation group FEV1,PEF,FEV1/FVC and 6MWT were higher than those in the control group,while CAT score,serum PI3K and Akt levels,gray value of PI3K and Akt protein expression,serum TNF-α,IL-8 and IL-15 levels lower than those in the control group,difference statistically significant(all P<0.05).Conclusion:Bufei Jianpi recipe combined with indatrogolone ammonium bromide has good effect on elderly patients with COPD in stable stage.It can improve lung function,quality of life,exercise endurance,down regulate expression of PI3K/Akt signal pathway,reduce cellular inflammatory response.
