摘要:
目的:研究疏风止咳汤联合糖皮质激素对咳嗽变异性哮喘患者血清白细胞介素8(IL-8)、呼出气一氧化氮(FeNO)水平的影响。方法:240例咳嗽变异性哮喘患者通过随机数字表法分为观察组和对照组,每组120例。对照组患者给予糖皮质激素布地奈德吸入气雾剂,观察组在对照组的基础上给予疏风止咳汤。观察两组治疗后临床疗效及不良反应发生情况,比较两组治疗前后中医症状积分、白细胞介素4(IL-4)、白细胞介素8(IL-8)、肿瘤坏死因子-α(TNF-α)、降钙素原(PCT)及呼出气一氧化氮(FeNO)、第一秒用力呼气量(FEV1)、用力肺活量(FVC)及最大呼气流量(PEF)。结果:治疗后,观察组总有效率86.67%高于对照组的70.83%,差异有统计学意义(P<0.05)。治疗后观察组咳嗽频率、口渴程度、咽痒程度、烦热程度及盗汗程度的积分低于对照组,差异有统计学意义(均P<0.05)。治疗后观察组IL-8、IL-4、TNF-α及PCT水平低于对照组,差异有统计学意义(均P<0.05)。治疗后观察组FeNO水平低于对照组,观察组FEV1、FVC及PEF高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组总不良反应率与对照组比较,差异无统计学意义(P>0.05)。结论:疏风止咳汤联合糖皮质激素能够降低咳嗽变异性哮喘患者血清IL-8、FeNO水平,且优于单一使用糖皮质激素的效果,能够更好地改善患者的肺功能,安全性较好。
Abstract:
Objective:To study the effects of Shufeng Zhike decoction combined with glucocorticoid on the levels of serum interleukin-8(IL-8)and fractional exhaled nitric oxide(FeNO)in cough variant asthma.Methods:240 patients with cough variant asthma were randomly divided into observation group and control group,120 cases in each group.Patients in the control group were given glucocorticoid budesonide inhalation aerosol,and the observation group were given Shufeng Zhike decoction on the basis of the control group.Before and after treatment,compared clinical efficacy and adverse reactions,the TCM syndrome score,interleukin-4(IL-4),interleukin-8(IL-8),tumor necrosis factor-α(TNF-α),procalcitonin(PCT),fractional exhaled nitric oxide(FeNO),forced expiratory volume in the first second(FEV1),forced vital capacity(FVC)and peak expiratory flow(PEF)between the two groups.Results:After treatment,the total effective rate in the observation group was 86.67%,which significantly higher than 70.83% in the control group,difference statistically significant(P<0.05).After treatment,the symptom scores of cough frequency,thirst,pharyngeal itching,fever and night sweat in the observation group were significantly lower than those in the control group,difference statistically significant(all P<0.05).After treatment,the levels of IL-8,IL-4,TNF-α and PCT in the observation group were significantly lower than those in the control group,difference statistically significant(all P<0.05).After treatment,the level of FeNO in the observation group was significantly lower than that in the control group,and the FEV1,FVC and PEF in the observation group were significantly higher than those in the control group,difference statistically significant(all P<0.05).After treatment,there was no significant difference in the total adverse reaction rate between the observation group and the control group(P>0.05).Conclusion:Shufeng Zhike decoction combined with glucocorticoid can significantly reduce the levels of serum IL-8 and FeNO in cough variant asthma.It's better than the effect of single use of glucocorticoid.It can better improve the pulmonary function of patients and safety.
