摘要:
目的:分析祛风活络丸联合郑氏伤科针灸对桡骨远端骨折并发创伤后急性骨萎缩患者疼痛及腕关节功能的影响。方法:选择110例桡骨远端骨折并发创伤后急性骨萎缩患者,按随机数字表法分两组,各组55例。两组均采用西医常规治疗,在此基础上对照组患者采用单纯郑氏伤科针灸治疗,研究组在对照组基础上,加服祛风活络丸,两组均治疗8周。比较两组临床疗效,治疗前、治疗后静息及活动的疼痛程度、关节僵硬、压痛、皮肤颜色改变评分,腕关节的掌屈、背伸、旋前以及旋后角度等关节活动度,腕关节功能及生活质量,以及治疗期间发生的不良反应。结果:治疗后,研究组的总有效率为96.36%高于对照组的78.18%,差异有统计学意义(P<0.05)。与治疗前比较,治疗后两组Cooney腕关节功能评分(疼痛、功能状况、活动度百分比、屈伸活动度、握力),Barthel指数(BI)评分,掌屈、背伸、旋前、旋后角度均升高,且研究组各项均高于对照组,差异有统计学意义(均P<0.05)。与治疗前比较,治疗后两组静息疼痛程度、活动疼痛程度、关节僵硬、压痛、皮肤颜色改变评分均降低,且研究组均低于对照组,差异有统计学意义(均P<0.05)。治疗期间研究组不良反应总发生率为7.27%,对照组为3.64%,组间比较差异无统计学意义(P>0.05)。结论:祛风活络丸联合郑氏伤科针灸治疗桡骨远端骨折并发创伤后急性骨萎缩临床效果较好,可减轻疼痛、关节诸症,提高关节活动度,改善腕关节功能及患者生活质量。
Abstract:
Objective:To analyze effects of Qufeng Huoluo pill combined with Zheng's traumatology acupuncture on pain and wrist function in patients with distal radius fracture complicated with post-traumatic acute bone atrophy.Methods:A total of 110 patients with distal radius fracture complicated with post-traumatic acute bone atrophy were included in the study group and the control group,with 55 cases in each group,by random number table method.Both groups received conventional western medicine treatment.The control group were received pure acupuncture treatment of Zheng's traumatology acupuncture,and the study group received Qufeng Huoluo pill on the basis of the control group.Both groups were treated for 8 weeks.The two groups of efficacy,pain degree during rest and activity,joint stiffness,tenderness,skin color change score,wrist flexion,back extension,pronation and supination angle and other joint activity,wrist function and quality of life before and after treatment,and adverse reactions during treatment were compared.Results:After treatment,the total effective rate was 96.36% in the study group higher than 78.18% in the control group,difference statistically significant(P<0.05).After treatment,scores of Cooney wrist function(pain,functional status,percentage of range of motion,flexion and extension range of motion,grip strength),Barthel index(BI),angles of palmarflexion,dorsiflexion,pronation and supination increased in both groups were higher than before treatment,and those indexs in the study group were higher than those in the control group,difference statistically significant(all P<0.05).After treatment,the scores of resting pain,activity pain,joint stiffness,tenderness and skin color change decreased in both groups were lower than before treatment,and those indexs in the study group were lower than those in the control group,difference statistically significant(all P<0.05).The incidence of adverse reactions in the study group during treatmen was 7.27% and 3.64% in the control group,there was no significant difference(P>0.05).Conclusion:Qufeng Huoluo pills combined with Zheng's traumatology acupuncture had definite effect on distal radius fracture complicated with post-traumatic acute bone atrophy,which could reduce pain and arthrosis symptoms,improve the range of motion,and finally improve the wrist function and quality of life.
