摘要:
胃食管反流病(GERD)是临床上较为常见的消化系统疾病,因其病程反复、迁延难愈等特点而严重地影响了患者的生活及心理。现阶段,GERD的发病机制存在复杂性和不确定性,笔者通过对近几年中西医病因病机及治疗相关文献收集整理,笔者发现西医认为GERD的发病机制与食管下括约肌(LES)抗反流屏障结构障碍、食管对胃反流物的廓清功能异常等相关,治疗以抑酸、促进胃肠动力及抗反流手术等为主。古今中医医家则认为在多因素下致肝胃不和、气机失和为其总病机,在治疗方面,通过辨证分型采用药物疗法、中医外治法治疗等。现笔者通过收集整理近几年国内外GERD文献资料,阐述了中西医学者对GERD的病因病机和治疗两方面的认识,以期为治疗GERD进展的研究提供参考。
Abstract:
Gastroesophageal reflux disease(GERD),which is a common digestive system disease in clinic,which seriously affects patients's life and psychology on account of its repeated course and difficult recovery.At present,the pathogenesis of GERD is complicated and uncertain.By collecting and sorting out the relevant literature about etiology,pathogenesis and treatment of traditional Chinese and western medicine in recent years.The author finds that western medicine thinks that the pathogenesis of GERD is related to the structural disorder of the lower esophageal sphincter(LES)anti-reflux barrier and the abnormal clearance function of esophagus to gastric reflux.And the treatments are mainly acid suppression,gastrointestinal motility promotion and anti-reflux surgery.Ancient and modern Chinese medicine doctors believe that disharmony between liver and stomach and disharmony between qi and blood are the general pathogenesis.In terms of treatment,drug therapy and external treatment of traditional Chinese medicine are adopted by syndrome differentiation and classification.By collecting and sorting out the literature of GERD at home and abroad in recent years,the author expounds the understanding of etiology,pathogenesis and treatment of GERD by Chinese and western scholars,in order to provide reference for the research on the progress of treating GERD.
