摘要:
目的:探究胃瘫外敷方敷贴中脘、神阙穴联合针灸治疗老年肿瘤患者胰十二指肠术后胃瘫的疗效。方法:选取113例老年肿瘤胰十二指肠术后胃瘫患者,依据随机数字表法将患者分为对照组(n=56)、观察组(n=57)。对照组患者予以针灸治疗,观察组患者在对照组基础上予以胃瘫外敷方敷贴中脘、神阙穴联合治疗。比较两组患者临床疗效、中医症候积分、胃肠功能改善相关指标、血红蛋白(Hb)、血清白蛋白(ALB)、转铁蛋白(TRF)。结果:观察组治疗总有效率高于对照组(P<0.05); 治疗后两组主症、次症以及中医症候总积分降低(均P<0.05),且观察组低于对照组(均P<0.05); 观察组治疗后肠鸣音恢复时间、腹胀感消失时间、首次排气时间、胃肠减压引流量及恢复正常饮食时间短于对照组(均P<0.05); 治疗后两组ALB、Hb、TFR升高(均P<0.05),且观察组高于对照组(均P<0.05)。两组患者治疗期间均未发生明显不良反应。结论:胃瘫外敷方敷贴中脘、神阙穴联合针灸治疗老年肿瘤患者胰十二指肠术后胃瘫疗效确切,可改善患者临床症状,改善患者营养状态,安全性高。
Abstract:
Objective:To investigate the effect of applying Weitan Waifu recipe patches at Zhongwan and Shenque points combined with acupuncture in the treatment of gastroplegia in elderly patients with tumors after pancreaticoduodenal surgery.Methods:A total of 113 elderly patients with tumors who developed gastroplegia after pancreaticoduodenal surgery were randomly assigned to the control group(n=56)and the observation group(n=57).Patients in the control group were treated with acupuncture,while those in the observation group were treated with Weitan Waifu recipe patches at Zhongwan and Shenque points combined with acupuncture.Clinical effect,TCM syndrome scores,gastrointestinal function related indicators,hemoglobin(Hb),serum albumin(ALB)and transferrin(TRF)were compared between the groups.Results:The treatment response rate in the observation group was significantly higher than that in the control group(P<0.05).After treatment,main symptom scores,secondary symptom scores and total TCM syndrome scores of the two groups decreased(all P<0.05),the observation group had lower scores than the control group(all P<0.05).After treatment,the recovery time of bowel sounds,disappearance time of abdominal distension,first exhaust time,drainage volume of gastrointestinal decompression and time of returning to normal diet in the observation group were significantly shorter than those in the control group(all P<0.05).After treatment,the levels of Hb,ALB and TRF in the two groups increased(all P<0.05),and the levels in the observation group were significantly higher than those in the control group(all P<0.05).No obvious adverse reactions occurred in the two groups during treatment.Conclusion:Applying Weitan Waifu recipe patches at Zhongwan and Shenque points combined with acupuncture is effective in the treatment of gastroparesis in elderly patients with tumors after pancreaticoduodenal surgery,which can alleviate the patients' clinical symptoms,improve their nutritional status,and has high safety.
