摘要:
目的:探讨电针治疗结合振动训练对缺氧缺血性脑病患儿运动功能的影响。方法:选取90例缺氧缺血性脑病患儿,按照随机数字表法将其分为对照组、电针组、联合组,每组30例。对照组采用常规康复技术治疗,电针组在此基础上进行电针治疗,联合组在对照组基础上采用电针结合振动训练的治疗方法。治疗前后,采用粗大运动功能评估量表(GMFM)、粗大运动功能分级系统(GMFCS)、Breg平衡量表(BBS)、表面肌电图肌电积分值(iEMG)及血清核转录因子κB(NF-κB)、B淋巴细胞瘤-2(Bcl-2)、Bcl-2相关X蛋白(Bax)含量检测对三组患儿的运动功能进行评定。结果:治疗后,与对照组比较,电针组及联合组患儿GMFM评分、GMFCS评分、BBS评分、iEMG值提高,差异有统计学意义(均P<0.05); 且联合组各项分值高于电针组,差异有统计学意义(均P<0.05)。与对照组比较,电针组、联合组血清NF-κB、Bax水平下降,Bcl-2水平升高(均P<0.05); 与电针组相比,联合组NF-κB、Bax水平降低,Bcl-2水平升高,差异有统计学意义(均P<0.05)。结论:电针治疗结合振动训练能明显改善缺氧缺血性脑病患儿的运动能力,其作用机制可能与调节NF-κB/Bcl-2、Bax信号通路,抑制细胞凋亡,减少肌肉萎缩,同时通过振动刺激加强肌纤维主动收缩,增强肌肉力量,从而提高骨骼肌运动功能有关。
Abstract:
Objective:To explore the effect of electroacupuncture combined with vibration training on motor function of children with hypoxic ischemic encephalopathy.Methods:90 children with hypoxic ischemic encephalopathy were selected and divided into control group,electroacupuncture group and combination group according to random number table method,with 30 cases each group.The control group was treated with conventional rehabilitation techniques,the electroacupuncture group was treated with electroacupuncture on this basis,and the combined group was treated with electroacupuncture combined with vibration training.Before and after treatment,the gross motor function assessment scale(GMFM),gross motor function rating system(GMFCS),breg balance scale(BBS),surface electromyography(iEMG)and serum nuclear transcription factor κB(NF-κB),B lymphoblastoma-2(Bcl-2),Bcl-2 associated X protein(Bax)in the three groups were compared.Results:After treatment,compared with the control group,the GMFM score,GMFCS score,BBS score,surface electromyography iEMG of the children in the electroacupuncture group and the combination group were significantly higher(all P<0.05),and the scores of the combination group were higher than those of the electroacupuncture group,and the difference was statistically significant(all P<0.05).Compared with the control group,the levels of NF-κB and Bax decreased and the levels of Bcl-2 increased in electroacupuncture group and combination group(all P<0.05).Compared with the electroacupuncture group,the level of NF-κB and Bax decreased significantly,while the level of Bcl-2 increased significantly in the combined group(all P<0.05).Conclusion:Electroacupuncture combined with vibration training can significantly improve the motor ability of children with hypoxic ischemic encephalopathy,and its mechanism may be related to the regulation of NF-κB/Bcl-2,Bax signal pathway,inhibit cell apoptosis,reducing muscle atrophy while enhancing active contraction of muscle fibers through vibration stimulation enhances muscle strength,thereby improving skeletal muscle motor function.
