摘要:
目的:探究针刺联合闪罐治疗对慢性非特异性腰痛(CNLBP)患者的治疗疗效及对患者灶点血流灌注量、表面肌电特征的影响。方法:选择CNLBP患者80例,采用不同治疗方式分为观察组(42例)和对照组(38例)。对照组患者行针刺治疗,观察组患者在对照组的基础上联合闪罐治疗。比较两组患者的治疗疗效,中医证候评分、表面肌电特征以及灶点血流灌注量变化情况。同时,采用腰痛评分标准(ODI)、视觉疼痛模拟评分法(VAS)评估疼痛程度。结果:观察组治疗总有效率高于对照组(P<0.05)。治疗后,观察组中医证候脉弦紧、腰骶压痛、硬结、舌暗紫评分均低于对照组(P<0.05)。治疗后,观察组竖脊/多裂肌的伸屈比(FRR)均大于对照组。治疗2周后,两组的 ODI评分比较差异有统计学意义(P<0.05),VAS评分比较无统计学差异(P>0.05); 治疗4周后,观察组 ODI、VAS评分均低于对照组(P<0.05)。治疗后,观察组肾俞、膀胱俞、大肠俞及腰椎3-4横突的血流灌注量均低于对照组(P<0.05)。结论:针刺联合闪罐治疗可有效改善CNLBP患者的临床症状,降低灶点血流灌注量、增强表面肌电信号,提高治疗疗效。
Abstract:
Objective:To explore the therapeutic effect of acupuncture combined with flash pot therapy on patients with chronic non-specific low back pain(CNLBP)and the influence on focal point blood perfusion volume and surface EMG characteristics.Methods:80 CNLBP patients were divided into an observation group(42 cases)and a control group(38 cases)using different treatment methods.Control group received acupuncture treatment,observation group on the basis of the control group combined with flash pot treatment.The therapeutic effect,TCM syndrome score,surface EMG characteristics and focal point blood perfusion volume were compared between the two groups.Meanwhile,the lower back pain scale(ODI)and visual pain analog scale(VAS)were used to evaluate the pain degree.Results:The total effective rate of observation group was higher than that of control group(P<0.05).After treatment,the scores of traditional Chinese medicine syndrome of pulse chord tightness,lumbosacral tenderness,indusion and tongue dark purple in the observation group were lower than those in the control group(P<0.05).After treatment,the extension/flexor ratio(FRR)of the erector spine/multifidus muscle in the observation group was greater than that in the control group.After 2 weeks of treatment,ODI scores in the observation group were lower than those in the control group(P<0.05).But there were no differences in VAS scores between the two groups(P>0.05).After 4 weeks of treatment,ODI and VAS scores in the observation group were lower than those in the control group(P<0.05).After treatment,the blood perfusion volume of the observation group was lower than that of the control group(P<0.05).Conclusion:Acupuncture combined with flash pot treatment can effectively improve the clinical symptoms of CNLBP patients,reduce the focal point blood perfusion volume,enhance surface electromyographic signal,improve therapeutic effect.
