摘要:
目的:探讨通督益脑化痰法在风痰阻络型缺血性脑卒中(IS)治疗中的应用价值及对胰岛素样生长因子-1(IGF-1)、脑源性神经营养因子(BDNF)及血管内皮生长因子(VEGF)的影响。方法:随机将120例IS(风痰阻络型)患者分为接受西药常规治疗的对照组及接受西药联合通督益脑化痰法治疗的观察组,每组60例。对比两组中医临床症状、神经功能损伤、血清神经递质、IGF-1、BDNF及VEGF变化情况。结果:治疗2周后,观察组与对照组相比,头晕、半身不遂、口眼歪斜、言语謇涩、感觉减退及总评分明显降低(P<0.05)。治疗2周后,观察组与对照组相比,NIHSS评分明显降低,MMSE评分及ADL评分明显升高(P<0.05)。治疗2周后,与对照组相比,观察组5-羟色胺(5-HT)、去甲肾上腺素(NE)和多巴胺(DA)、BDNF及VEGF水平明显升高,IGF-1明显降低(P<0.05)。观察组的总有效率与对照组相比明显提高(P<0.05)。结论:在IS(风痰阻络型)的治疗中,通督益脑化痰法可有效提高患者的治疗效果,改善患者的IGF-1、BDNF及VEGF水平。
Abstract:
Objective:To investigate the application value of Tongdu Yinao Huatan method in the treatment of wind-phlegm-stasis type ischemic stroke and its investigate the effect on IGF-1,BDNF and VEGF.Methods:A total of 120 patients with ischemic stroke(wind-phlegm-stasis type)were randomly divided into control group receiving conventional western medicine treatment and observation group receiving traditional Chinese medicine decoction under the guidance of Tongdu Yinao Huatan method,with 60 cases in each group.Traditional Chinese medicine clinical symptoms,neurological function injury,serum neurotransmitter,IGF-1,BDNF and VEGF changes were compared between the two groups.Results:After 2 weeks of treatment,compared with the control group,the observation group showed a significant decrease in dizziness,hemiplegia,mouth and eye deviation,speech difficulty,decreased sensation,and overall score(P<0.05).After 2 weeks of treatment,compared with the control group,the NIHSS score in the observation group was significantly reduced,while the MMSE score and ADL score were significantly increased(P<0.05).After 2 weeks of treatment,compared with the control group,the levels of 5-HT,NE,DA,BDNF,and VEGF in the observation group were significantly increased,while IGF-1 was significantly reduced(P<0.05).The total effective rate of the observation group was significantly improved compared to the control group(P<0.05).Conclusion:In the treatment of ischemic stroke(wind-phlegm-stasis type),Tongdu Yinao Huatan method can effectively improve the therapeutic effect of patients,improve the levels of IGF-1,BDNF and VEGF.
