摘要:
目的:探讨手法复位夹板外固定联合活血行气散对儿童尺桡骨中段骨折术后肿胀、疼痛和腕关节功能的影响。方法:选取157例尺桡骨中段骨折患儿作为研究对象,根据信封法将其随机分为中药组79例和对照组78例。对照组予以手法复位夹板外固定治疗,中药组在对照组的基础上另予活血行气散治疗。比较两组患儿治疗前后肿胀程度、疼痛度、腕关节功能、炎症因子水平、中医症候积分及治疗后临床疗效。结果:中药组治疗后肿胀程度、视觉模拟评分法(VAS)评分均明显低于对照组(P<0.05),而改良Mayo评分显著高于对照组(P<0.05); 中药组治疗后C反应蛋白(CRP)、白介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)水平明显低于对照组(P<0.05); 中药组治疗后肿胀、刺痛、活动受限及瘀斑症候积分均明显低于对照组(P<0.05); 中药组总有效率为91.14%,对照组为76.92%,中药组总有效率显著高于对照组(P<0.05)。结论:手法复位夹板外固定联合活血行气散治疗儿童尺桡骨中段骨折,可有效缓解术后肿胀及疼痛,促进腕关节功能恢复,降低炎症因子水平。
Abstract:
Objective:This study was conducted to investigate the effect of manual reduction and splint external fixation combined with Huoxue Xingqi powder on postoperative swelling,pain and wrist function in children with middle ulnar and radial fractures.Methods:A total of 157 children with middle ulnar and radial fractures were selected as the research subjects.They were randomly divided into the traditional Chinese medicine group(79 cases)and the control group(78 cases)by the envelope method.Patients in the control group were treated with manual reduction and splint external fixation,and patients in the traditional Chinese medicine group were treated with Huoxue Xingqi powder based on the treatment of control group.Swelling degree,pain degree,wrist function,inflammatory factor levels,traditional Chinese medicine syndrome scores,and clinical efficacy were compared between the two groups.Results:After treatment,swelling degree and Visual Analogue Scale(VAS)score of the traditional Chinese medicine group were significantly milder/lower than those of the control group(P<0.05).The modified Mayo score was significantly higher than that of the control group(P<0.05).The levels of C-reactive protein(CRP),interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)were significantly lower than those in the control group(P<0.05).The scores for swelling,prickling,limitation of motion,and ecchymosis of the traditional Chinese medicine group were significantly lower than those of the control group(P<0.05).The total effective rate in the traditional Chinese medicine group(91.14%)was significantly higher than that in the control group(76.92%,P<0.05).Conclusion:Applying manual reduction and splint external fixation combined with Huoxue Xingqi powder in the treatment of children with middle ulnar and radial fractures can effectively alleviate postoperative swelling and pain,promote the recovery of wrist function,and lower the levels of inflammatory factors.
