摘要:
目的:探究清胆解毒汤合四逆散治疗急性胰腺炎肝胆湿热证的临床效果。方法:选择102例肝胆湿热型急性胰腺炎患者,采用随机数字表法分成常规组(n=51)与中药组(n=51)。常规组接受液体治疗、镇痛及营养支持等常规治疗,中药组在此基础上加用清胆解毒汤合四逆散口服治疗。观察两组患者的中医证候积分、血清生化指标、症状恢复情况、临床疗效、不良反应发生情况及病死率。结果:治疗后,中药组中医证候积分、可溶性细胞间黏附分子-1(sICAM-1)、C反应蛋白(CRP)、淀粉酶(AMY)、脂肪酶(LPS)、肠抑胃肽(GIP)、胃肠激素活动性肠肽(VIP)水平均低于常规组(P<0.05); 中药组患者腹胀、腹痛消失时间与AMY、体温恢复正常时间均短于常规组(P<0.05); 中药组治疗总有效率高于常规组(P<0.05); 两组不良反应总发生率与病死率差异无统计学意义(P>0.05)。结论:清胆解毒汤合四逆散治疗急性胰腺炎肝胆湿热证能够降低中医证候积分,缓解临床症状,减轻炎症反应,改善肠道功能,提高临床疗效,且安全性较高。
Abstract:
Objective:To explore the clinical effect of Qingdan Jiedu decoction combined with Sini powder in the treatment of acute pancreatitis with liver and gallbladder damp-heat syndrome.Methods:A total of 102 patients with acute pancreatitis of liver and gallbladder damp-heat type were selected and divided into conventional group(n=51)and traditional Chinese medicine group(n=51)by random number table method.The conventional group received routine treatment such as liquid therapy,analgesia and nutritional support,on this basis,the TCM group received Qingdan Jiedu decoction and Sini powder oral treatment.The TCM syndrome score,serum biochemical indexes,symptom recovery,clinical efficacy,adverse reactions and mortality of the two groups were observed.Results:After treatment,the TCM syndrome score,soluble intercellular adhesion molecule-1(sICAM-1),C-reactive protein(CRP),amylase(AMY),lipase(LPS),gastric inhibitory peptide(GIP)and gastrointestinal hormone active intestinal peptide(VIP)levels in the TCM group were lower than those in the conventional group(P<0.05).The disappearance time of abdominal distension and abdominal pain,AMY and body temperature recovery time in the TCM group were shorter than those in the conventional group(P<0.05).The total effective rate of the TCM group was higher than that of the conventional group(P<0.05).There was no significant difference in the total incidence of adverse reactions and mortality between the two groups(P>0.05).Conclusion:Qingdan Jiedu decoction combined with Sini powder in the treatment of acute pancreatitis with liver and gallbladder damp-heat syndrome can reduce TCM syndrome score,relieve clinical symptoms,reduce inflammatory reaction,improve intestinal function,improve clinical efficacy,and have high safety.
