摘要:
目的:探讨二参真武汤对心肾阳虚型慢性心力衰竭(CHF)大鼠心肌能量代谢的影响。方法:60只大鼠随机选取8只设为空白组,剩余52只通过甲状腺切除后给予0.02%阿霉素腹腔注射建立CHF模型,将建模大鼠随机分为模型组、阳性对照组、二参真武汤低剂量组、二参真武汤高剂量组,每组8只,连续灌胃28 d。给药结束后,ELISA法测空白组、模型组血清三碘甲状腺原氨酸(T3)、甲状腺素(T4)、氨基末端B型利钠肽(NT-proBNP)和各组心肌组织腺嘌呤核苷三磷酸(ATP)、腺嘌呤核苷二磷酸(ADP)、B型脑钠肽(BNP)、肌酸激酶同工酶(CK-MB)、Na+-K+-ATP酶、Ca2+-ATP酶。彩色多普勒心脏超声检测心功能; HE染色法观察心脏病理形态; 天狼星染色法观察心肌胶原纤维沉积形态; 电镜观察心肌线粒体超微结构。结果:造模后大鼠血清NT-proBNP上升,T3 、T4 下降(均P<0.01)。与空白组相比,模型组大鼠左心室射血分数(LVEF)、左心室短轴缩短率(LVFS)下降,左室收缩末期内径(LVIDs)、左心室收缩末期容积(LVESV)上升(均P<0.01)。ATP、ADP、Na+-K+-ATP酶、Ca2+-ATP酶下降,BNP、CK-MB上升(均P<0.01 )。心肌细胞肿胀,边缘模糊,胶原纤维沉积增多,线粒体数量减少、嵴模糊。与模型组相比,阳性对照组及二参真武汤低剂量组、二参真武汤高剂量组 LVEF、LVFS上升、LVIDs、LVESV下降,ATP、ADP、Na+-K+-ATP酶、Ca2+-ATP酶上升,BNP、CK-MB下降(均P<0.01); 肿胀心肌细胞减少,胶原纤维沉积面积缩小,心肌线粒体损伤减轻。结论:二参真武汤可有效改善CHF大鼠心肌损伤,对心脏有保护作用,其机制可能是通过调节ATP、ADP改善心肌能量代谢。
Abstract:
Inquire into impact of Ershen Zhenwu decoction on cardiac muscle energy metabolism in ratswith chronic heart failure(CHF)due to Xinshen Yangxu.Methods:Eight rats from 60 rats were stochastically selected as blank group,and the remaining 52 rats were rendered 0.02% Doxorubicin(DOX)intraperitoneal injection after thyroidectomy to establish CHF model of Xinshen Yangxu.The model rats were stochastically split into model group,positive control group,Ershen Zhenwu low-dose group and Ershen Zhenwu high-dose group,with 8 rats in each group.All groups were given corresponding drugs,and course of treatment was 28 days.End of drug administration,serum triiodothyronine(T3),thyroxine(T4),N-terminal B-type natriuretic peptide(NT-proBNP)in blank group and model group.The myocardial Adenine nucleoside triphosphate(ATP),adenosine diphosphate(ADP),B-type natriuretic peptide(BNP),creatine kinase isoenzyme(CK-MB),Na+-K+-ATPase,Ca2+-ATPase each group were measured by ELISA.Cardiac function were detected by color doppler echocardiography.Pathological changes of heart were observed by HE staining.The morphology of myocardial collagen fiber deposition were observed by Sirius staining.Ultrastructure of myocardial mitochondriawas observed by electronmicroscope.Results:After modeling,serum NT-proBNP increased and T3 and T4 decreased(P<0.01).Compared with the blank group,left ventricular ejection fraction(LVEF)and left ventricular short axis shortening rate(LVFS)decreased in the model group,while left ventricular end-systolic inner diameter(LVIDs)and left ventricular end-systolic volume(LVESV)increased(all P<0.01).ATP,ADP,Na+-K+-ATPase and Ca2+-ATPase decreased,while BNP and CK-MB increased(all P<0.01).The myocardial cells were swollen,the edge was vague,increase of collagen fiber deposition,mitochondria decreased and the crista was vague.Compared with the model group,LVEF and LVFS in the high and the low dosegroups of Ershen Zhenwu decoction and the positive control group increased,while LVIDs and LVESV decreased,while ATP,ADP,Na+-K+-ATPase and Ca2+-ATPase increased,BNP and CK-MB decreased(all P<0.01).Swelling myocardial cellsdecreased,collagen fibersdeposition surface area decreased,and myocardial mitochondrial harm was alleviated.Conclusion:Ershen Zhenwu decoction can validly amend myocardial injury and protect heart in CHF rats,and its mechanism may be related to regulate and control ATP and ADP energy metabolism.
