摘要:
目的:研究增液承气汤对脓毒症肠功能障碍大鼠炎症反应、免疫及肠功能的影响。方法:将大鼠随机分为五组,每组20只,除假手术组外,其余各组建立脓毒症模型。各组分别进行对应药物干预后,ELISA检测白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、分泌型免疫球蛋白(SIgA)水平。HE染色观察大鼠肠道病理改变。流式细胞术检测大鼠胸腺、脾脏及外周血淋巴细胞凋亡情况。结果:CLP模型组IL-6、TNF-α水平较假手术组明显升高,增液承气汤各剂量组IL-6、TNF-α水平较CLP模型组明显下降,以增液承气汤高剂量组下降更显著(均P<0.05)。CLP模型组SigA水平较假手术组下降,差异有统计学意义(P<0.05); 增液承气汤低、中剂量组SigA水平比较差异无统计学意义(P>0.05); 增液承气汤高剂量组血清SIgA水平高于CLP模型组(P<0.05)。CLP模型组可见肠上皮变性、脱落,黏膜下层坏死; 增液承气汤中、高剂量组可见肠上皮变性,绒毛较完整,炎性细胞比CLP组少。与CLP模型组比较,增液承气汤中、高剂量组Chiu's评分下降(均P<0.05)。与假手术组对比,CLP模型组胸腺、脾脏淋巴细胞凋亡水平明显上升(均P<0.05); 与CLP组对比,增液承气汤各剂量组外周血、胸腺、脾脏淋巴细胞凋亡水平下降(均P<0.05)。结论:增液承气汤可改善CLP大鼠肠道损伤,减轻炎症反应,提高SIgA水平,抑制淋巴细胞凋亡。
Abstract:
Objective:To study the effects of Zengye Chengqi decoction on inflammation,immunity and intestinal function in rats with septic intestinal dysfunction.Methods:The rats were randomly divided into 5 groups,with 20 rats in each group.Sepsis model was established in all groups except the sham operation group.The levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and secreted immunoglobulin(SIgA)in each group were detected by ELISA after corresponding drug intervention.The pathological changes of intestinal tract were observed by HE staining.The apoptosis of thymus,spleen and peripheral blood lymphocytes was detected by flow cytometry.Results:The levels of IL-6 and TNF-α in CLP model group were significantly higher than those in sham operation group,and the levels in Zengye Chengqi decoction groups were significantly lower than those in CLP model group,especially in Zengye Chengqi decoction high-dose group(all P<0.05).The SigA level in CLP model group was lower than that in sham operation group,the difference was statistically significant(P<0.05).There was no significant difference in SigA level between Zengye Chengqi decoction low dose and medium dose groups(P>0.05).Serum SIgA level in Zengye Chengqi decoction high dose group was higher than that in CLP model group(P<0.05).In CLP model group,intestinal epithelial degeneration,exfoliation and submucosal necrosis were observed.Epithelial degeneration,villi were more complete and inflammatory cells in Zengye Chengqi decoction group were less than CLP model group.Compared with CLP model group,the Chiu's scores in Zengye Chengqi decoction medium and high dose groups were decreased(P<0.05).Compared with sham operation group,the apoptosis level of thymus and spleen lymphocytes in CLP model group was significantly increased(all P<0.05).Compared with CLP group,the apoptosis levels of lymphocytes in peripheral blood,thymus and spleen of Zengye Chengqi decoction groups were decreased(all P<0.05).Conclusion:Zengye Chengqi decoction can improve intestinal injury,reduce inflammation reaction,increase SIgA level and inhibit lymphocyte apoptosis in CLP rats.
