摘要:
目的:探讨盐酸戊乙奎醚联合消瘀宽胸汤对胸部创伤大鼠T辅助细胞(Th)1/2细胞因子、昼夜节律蛋白及肠屏障功能的影响。方法:将50只大鼠随机分为正常组、模型组、盐酸戊乙奎醚组、消瘀宽胸汤组及联合组,每组10只。除正常组外,其余组大鼠均建立胸部器官组织损伤,出血严重模型,盐酸戊乙奎醚组、联合组分别在麻醉前肌肉注射0.5 mg盐酸戊乙奎醚,消瘀宽胸汤组、联合组分别给予10 ml/kg的消瘀宽胸汤灌胃,正常组、模型组注射0.9%氯化钠溶液。采用双抗体夹心酶联免疫吸附(ELISA)法,观察干扰素(IFN)-γ水平和白介素(IL)-4水平变化; 采用血液流变学检测全血黏度、血浆黏度、血小板聚集率、血沉指标变化; 在光镜下观察肠屏障功能指标; 免疫印迹法检测胸部组织中昼夜自发输出周期蛋白(Clock)、芳香烃受体核转录蛋白样1抗体(Bmal1)、香草素受体4(TRPV4)蛋白含量; 苏木精-伊红染色观察胸部组织标本。结果:与正常组大鼠相比,模型组大鼠IFN-γ升高、IL-4降低; 全血黏度、血浆黏度、血小板聚集率、血沉均升高; 空肠内肠绒毛高度、绒毛中段直径、隐窝深度、绒毛表面积均降低; Clock蛋白、Bmal1蛋白降低,TRPV4蛋白升高(P<0.05)。与模型组大鼠比较,盐酸戊乙奎醚组大鼠、消瘀宽胸汤组大鼠血清中的IFN-γ降低,IL-4升高; 全血黏度、血浆黏度、血小板聚集率、血沉均降低; 空肠内的肠绒毛高度、绒毛中段直径、隐窝深度、绒毛表面积均升高; Clock蛋白、Bmal1蛋白升高,TRPV4蛋白降低(P<0.05)。盐酸戊乙奎醚组大鼠与消瘀宽胸汤组大鼠比较差异无统计学意义(P>0.05)。与消瘀宽胸汤组大鼠比较,联合组大鼠血清中的IFN-γ降低,IL-4升高; 全血黏度、血浆黏度、血小板聚集率、血沉降低; 空肠内的肠绒毛高度、绒毛中段直径、隐窝深度、绒毛表面积升高; Clock蛋白、Bmal1蛋白升高,TRPV4蛋白降低(P<0.05)。结论:盐酸戊乙奎醚联合消瘀宽胸汤可以改善胸部创伤大鼠Th1/2细胞因子平衡,调节肠屏障功能障碍及昼夜节律失衡,对胸部损伤具有活血化瘀的功效。
Abstract:
Objective:To investigate the effects of penehyclidine hydrochloride combined with Xiaoyu Kuanxiong decoction on T helper cell(Th)1/2 cytokines,circadian rhythm proteins and intestinal barrier function in rats with chest trauma.Methods:Fifty rats were randomly divided into normal group,model group,penehyclidine hydrochloride group,Xiaoyu Kuanxiong decoction group and combined group,with 10 rats in each group.The model of chest organ tissue injury and severe bleeding was established in all the other groups except the normal group.Penehyclidine hydrochloride group and combined group were intramusculally injected with 0.5 mg penehyclidine hydrochloride before anesthesia,Xiaoyu Kuanxiong decoction group and combined group were given 10 ml/kg Xiaoyu Kuanxiong decoction by intragastric administration,and normal group and model group were injected with 0.9% sodium chloride solution.Double antibody sandwich enzyme-linked immunosorbent assay(ELISA)was used to observe the changes of interferon(IFN-γ)and interleukin(IL-4)levels.Blood viscosity,plasma viscosity,platelet aggregation rate and sedimentation rate were measured by hemorheology.The intestinal barrier function indexes were observed under light microscope.The contents of circadian cycle protein(Clock),aromatic hydrocarbon receptor nuclear transcription protein-like antibody(Bmal1)and vanillin receptor 4(TRPV4)were detected by Western blotting.Hematoxylin-eosin staining was used to observe the chest tissue samples.Results:Compared with normal group,IFN-γ increased and IL-4 decreased in model group.whole blood viscosity,plasma viscosity,platelet aggregation rate and erythrocyte sedimentation rate were increased.The villi height,middle villi diameter,crypt depth and villi surface area of jejunum were all decreased.Clock protein and Bmal1 protein were decreased,TRPV4 protein was increased(P<0.05).Compared with model group,the serum IFN-γ in penehyclidine hydrochloride group and Xiaoyu Kuanxiong decoction group decreased and IL-4 increased; whole blood viscosity,plasma viscosity,platelet aggregation rate and sedimentation rate were all decreased.The villi height,middle villi diameter,crypt depth and villi surface area in jejunum were all increased.Clock protein and Bmal1 protein were increased,TRPV4 protein was decreased(P<0.05).There was no significant difference between penehyclidine hydrochloride group and Xiaoyu Kuanxiong decoction group(P>0.05).Compared with Xiaoyu Kuanxiong decoction group,serum IFN-γ decreased and IL-4 increased in combined group.Whole blood viscosity,plasma viscosity,platelet aggregation rate,sedimentation rate decreased; the villi height,middle villi diameter,crypt depth and villi surface area in jejunum were increased.Clock protein and Bmal1 protein were increased,TRPV4 protein was decreased(P<0.05).Conclusion:Penehyclidine hydrochloride combined with Xiaoyu Kuanxiong decoction can improve the balance of Th1/2 cytokines,intestinal barrier dysfunction,circadian rhythm imbalance,chest injury,and has a good effect of promoting blood circulation and removing blood stasis in rats with chest trauma.
