摘要:
目的:探讨十三味消肿散外敷联合股骨近端防旋髓内针内固定术(PFNA)治疗老年股骨粗隆间骨折临床疗效。方法:选取165例老年股骨粗隆间骨折患者为研究对象,根据随机数字表法将其分为对照组、治疗组。对照组82例给予PFNA治疗,治疗组83例在对照组基础上予以十三味消肿散外敷。比较两组患者治疗前后患肢肿胀程度、疼痛度、髋关节功能、炎症因子水平、证候积分、并发症发生率。结果:治疗组与对照组术后肿胀、疼痛VAS评分较治疗前均下降,差异有统计学意义(P<0.05)。治疗组肿胀程度、疼痛VAS评分低于对照组,差异有统计学意义(均P<0.05)。治疗组术后7 d、1个月、1年髋关节功能Harris评分优于对照组,差异有统计学意义(均P<0.05)。治疗组治疗后炎症因子超敏C反应蛋白、白介素6低于对照组(均P<0.05)。治疗组治疗后局部瘀斑、刺痛、肿胀证候积分低于对照组(均P<0.05)。两组患者治疗后深静脉血栓、肺部感染、泌尿系感染、褥疮等并发症发生率比较,差异无统计学意义(均P>0.05)。结论:在PFNA基础上联合十三味消肿散外敷能提高老年股骨粗隆间骨折疗效,有效缓解患肢术后疼痛及肿胀,促进髋关节功能恢复,降低炎症因子水平。
Abstract:
Objective:To investigate clinical efficacy of external application of thirteen ingredient Xiaozhong powder combined with PFNA in treatment of intertrochanteric fracture of femur in the elderly.Methods:165 elderly patients with intertrochanteric fracture of femur were selected as research object,and randomly divided into control group and treatment group according to randomized numerical table method.82 cases in the control group were treated with proximal femoral nail anti-rotation(PFNA),and 83 cases in the treatment group were treated with external application of thirteen ingredient Xiaozhong powder on the basis of the control group.Compared degree of swelling,pain,hip function,inflammatory factor level,symptom score and complication rate of the affected limbs before and after the treatment of the two groups.Results:Postoperative swelling and pain VAS scores decreased in the observation and control groups compared before treatment,difference statistically significant(all P<0.05).The degree of swelling and pain VAS score decreased more significantly in the treatment group than in the control group,difference statistically significant(all P<0.05).The Harris score of hip joint function in the treatment group were better than those in the control group at 7 days,1 month and 1 year after surgery,difference statistically significant(all P<0.05).The levels of inflammatory factors hypersensitive C-reactive protein and interleukin 6 were significantly lower in the treatment group than in the control group after treatment(all P<0.05).Local ecchymosis,tingling and swelling symptom scores were significantly lower in the treatment group than those in the control group after treatment(all P<0.05).There was no significant difference in the incidence of complications such as deep vein thrombosis,pulmonary infection,urinary tract infection and decubitus ulcers between the two groups after treatment(P>0.05).Conclusion:The clinical efficacy of the addition of thirteen ingredient Xiaozhong powder on the basis of PFNA in the treatment of elderly intertrochanteric femur fracture was remarkable,which can effectively alleviate postoperative pain and swelling of affected limb,promote the recovery of the hip joint function,and reduce the level of inflammatory factors.
