摘要:
目的:观察哈氏育阴汤治疗肝肾阴虚型卵巢储备功能减退(DOR)的临床疗效及其对血管生成因子的影响。方法:纳入肝肾阴虚型DOR患者60例,采用随机数字表法分为治疗组和对照组,各30例。治疗组予哈氏育阴汤,对照组予坤泰胶囊,治疗3个月。观察两组患者的临床疗效,治疗前后中医证候评分、血清激素水平[基础卵泡刺激素(bFSH)、基础雌二醇(bE2)、抗缪勒管激素(AMH)、bFSH/基础黄体生成素(bLH)]、双侧卵巢窦卵泡数(AFC)、血管生成因子[缺氧诱导因子-1α(HIF-1α)、血管内皮生长因子(VEGF)、血管生成素-1(Ang-1)、Ang-2]的变化。结果:最终完成56例,治疗组29例,脱落1例; 对照组27例,脱落3例。治疗组总有效率(89.66%)高于对照组(74.07%)(P<0.05)。两组治疗后中医证候评分、bFSH、HIF-1α较治疗前下降,bE2较治疗前升高(均P<0.05); 治疗组治疗后AMH、AFC、VEGF、Ang-2、Ang-1较治疗前升高,bFSH/bLH较治疗前下降(均P<0.05); 且治疗组在改善中医证候评分、bFSH、bFSH/bLH、AMH、AFC、VEGF、Ang-2、Ang-1优于对照组(均P<0.05)。结论:哈氏育阴汤治疗肝肾阴虚型DOR有效,可改善患者中医临床症状,调节生殖激素及AMH水平,增加AFC,调节血管生成因子,提高卵巢储备功能。
Abstract:
Objective:To observe the clinical efficacy of diminished ovarian reserve(DOR)with liver-kidney yin deficiency treated by Ha's Yuyin decoction,and its effect on angiogenesis factor.Methods:According to the inclusion and exclusion criteria,60 patients with liver-kidney yin deficiency DOR were selected and randomly divided into treatment group(30 cases)and control group(30 cases).The treatment group was treated with Ha's Yuyin decoction,while the control group with Kuntai capsules.After three months,the clinical efficacy of the two groups were observed.The changes of TCM syndrome score,serum hormone level(bFSH,bE2,AMH,bFSH/bLH),AFC,angiogenesis factor(HIF-1α,VEGF,Ang-1,Ang-2)before and after treatment were observed and compared between the two groups.Results:Finally 56 cases were completed,29 cases in the treatment group and 27 cases in the control group.The total treatment response rate in the treatment group(89.66%)was significantly higher than that in the control group(74.07%)(P<0.05).TCM syndrome score,bFSH and HIF-1α decreased after treatment,while bE2 increased after treatment(all P<0.05).After treatment,AMH,AFC,VEGF,Ang-2 and Ang-1 were increased,while bFSH/bLH were decreased(all P<0.05).Moreover,the treatment group was better than the control group in improving TCM syndrome score,bFSH,bFSH/bLH,AMH,AFC,VEGF,Ang-2 and Ang-1(all P<0.05).Conclusion:Ha's Yuyin decoction has a significant therapeutic effect on DOR with liver-kidney yin deficiency,and can improve the clinical symptoms of traditional Chinese medicine,regulate reproductive hormones and AMH levels,increase AFC,regulate angiogenic factors,thereby improving ovarian reserve.
