摘要:
胃癌前病变(PLGC)是指胃黏膜上皮向胃癌转变的病理过程,包括慢性萎缩性胃炎及其基础上的肠上皮化生、异型增生和原位癌。现代中医认为,阻断“炎-癌”转化的关键环节就是调畅脏腑气机升降,而这与“圆运动”理论不谋而合。从圆运动之“中气如轴,四维如轮”的气机升降角度阐述PLGC的中医病因病机,顺应圆运动的理论内涵,以“调理中焦,兼顾四维,运轴行轮,运轮复轴”为指导思想辨治PLGC,旨在为其治疗和预防提供新的思路和方法。
Abstract:
Precancerous lesions of gastric cancer(PLGC)represent the pathological process of gastric mucosal epithelial transformation to gastric cancer.This includes chronic atrophic gastritis and its underlying intestinal epithelial hyperplasia,heterogeneous hyperplasia,and carcinoma in situ.In accordance with the tenets of modern Chinese medicine,the pivotal point in preventing the transformation of “inflammation-cancer” is the regulation of the Qi elevation of internal organs,which is in alignment with the theory of “circular motion”.This paper elucidates the etiology and pathogenesis of PLGC in traditional Chinese medicine from the perspective of qi elevation and the circular movement of the four dimensions,which is conceptualized as “the middle jiao is like an axis,and the four dimensions are like wheels”.The treatment of PLGC is based on the guiding principle of “regulating the middle jiao,taking into consideration of the four dimensions,transporting axes to move wheels,and transporting wheels to repeat the axes”.The objective of this study is to present novel approaches to the treatment and prevention of precancerous lesions of gastric cancer.
