摘要:
结直肠癌是常见的消化道恶性肿瘤。炎症被认为是导致大肠癌的主要原因之一,其产生的炎症微环境显著增加了结直肠癌的发病风险。故改善肿瘤炎症微环境,减缓“炎-癌转化”进程,对结直肠癌的防治有重要意义。中医火热毒邪与现代医学中的炎症关系密切,基于中医“取象比类”思维及临床医学研究,认为“亢火害物”可用来描述“炎-癌转化”的发展过程。“亢火”作为“炎-癌转化”发生的基础贯穿始终,“害物”则是其形成的根本原因。亢盛之火蓄积,一方面熏蒸肠腑衍生气滞、痰浊、瘀毒等邪实,另一方面伤害正气,损耗精血,使人体处于虚损状态而不能祛邪于外,最终因火成毒致虚、因虚生火聚毒成恶性循环,推动了“炎-癌转化”的进展。基于此,治疗应遵循中医“亢害承制”思想,纠人体正邪之偏颇,补益气血之虚,清降火性毒邪之实,减轻炎性浸润,防止“炎症-发育不良-癌”转化序列的形成,从而有效降低结直肠癌的发病率。
Abstract:
Colorectal cancer is a common malignant tumor of the gastrointestinal tract.Inflammation is considered to be one of the main causes of colorectal cancer,and the inflammatory microenvironment it generates significantly increases the risk of colorectal cancer.Therefore,improving the inflammatory microenvironment of tumors and slowing down the process of “inflammation-cancer transformation” are of great significance to the prevention and treatment of colorectal cancer.According to Chinese medicine,fire-heat toxicity is closely related to inflammation in modern medicine.Based on the analogy by appearance of Chinese medicine and modern medical research,the author believes that “raging inferno and injuring wu” can be used to describe the development process of “inflammation cancer transformation”.“Raging inferno” is the basis for the development of “inflammation cancer transformation” throughout the whole process,and “injuring wu” is the root cause of its formation.Hyperactive fire accumulation on the one hand can be transformed into poison fumigation bowel derived from it,on the other hand,the harm loss of positive qi,so that the human body is in a state of deficiency and can not get rid of the evil in the outside,which makes the fire become toxic to the deficiency,and the fire and toxicity due to the deficiency into a vicious circle,accelerates the process of “inflammation cancer transformation”.Therefore,for the treatment of “Kang hai cheng zhi” should take care of both positive and negative aspects,replenish the deficiency of human body's qi and blood,clear and reduce the reality of fire and toxicity,reduce the inflammatory infiltration,prevent the formation of “inflammation-dysplasia-cancer” transformation sequence,so as to effectively reduce the development of colorectal cancer.
