摘要:
膝骨关节炎(KOA)是一种以关节软骨进行性退变和关节炎症为病理特征的慢性关节疾病,其发病与年龄、关节力学失稳及过度机械负荷三个主要因素有关,早期主要表现为受累关节不同程度的肿痛、活动受限,严重影响患者的工作和生活,给个人和社会带来了沉重的经济负担,KOA的发病机制目前还不完全清楚,但与生物学因素密切相关,其中炎症信号通路的激活起到关键作用。以往的研究认为IL-1β/p38MAPK信号通路是早期KOA炎性信号通路的研究热点,该信号通路不仅可以调控软骨细胞的退变、凋亡及降解,还能逆向调控关节微环境中的炎性反应,在KOA的病理发展过程中起着至关重要的作用。综述近5年温针灸通过调控IL-1β/p38MAPK信号通路治疗KOA的成果,进一步为临床运用温针灸治疗KOA提供相关基础研究依据。
Abstract:
Knee osteoarthritis (KOA) is a chronic joint disease characterized by progressive degeneration of articular cartilage and joint inflammation,and its onset is related to three main factors:age,joint mechanical instability and excessive mechanical load.The activation of inflammatory signaling pathways plays a key role.Previous studies have suggested that the IL-1β/p38MAPK signaling pathway is a research hotspot in the early stage of KOA inflammatory signaling pathway,which can not only regulate the degeneration,apoptosis and degradation of chondrocytes,but also reverse regulate the inflammatory response in the joint microenvironment,which plays a vital role in the pathological development of KOA.This paper reviews the results of warm acupuncture in the treatment of KOA by regulating the IL-1β/p38MAPK signaling pathway in the past five years,and further provides relevant basic research basis for the clinical application of warm acupuncture in the treatment of KOA.
