摘要:
目的：研究关节活络膏联合关节镜下微骨折术对中后期膝骨关节炎（KOA）软骨损伤修复的临床疗效。方法：选择中后期KOA寒湿痹阻证患者40例，随机分为试验组、对照组。试验组治疗方案为关节活络膏联合关节镜下微骨折术，对照组治疗方案为单纯关节镜下微骨折术。通过观察两组治疗前及术后6个月的视觉模拟评分（VAS）、膝关节活动度（ROM）及骨关节炎指数（WOMAC）评分，测量膝关节正位片关节间隙高度及磁共振成像（MRI）软骨厚度，分析两组治疗方法对软骨修复的疗效差异。结果：试验组与对照组组内比较，治疗后VAS 评分、WOMAC评分较治疗前均降低，差异有统计学意义（均P＜0.01）。两组患者治疗后膝关节ROM较治疗前均增加，差异有统计学意义（均P＜0.05）。治疗后两组VAS、WOMAC、ROM比较，差异有统计学意义（均P＜0.05）。膝关节关节间隙、软骨厚度数值治疗后较治疗前均有提升，差异有统计学意义（均P＜0.01）。治疗后两组比较，试验组优于对照组，差异有统计学意义（均P＜0.05）。结论：关节活络膏联合关节镜下微骨折术对中后期膝骨关节炎软骨损伤修复的临床疗效较好，优于单纯关节镜下微骨折术对软骨的修复效果。
Abstract:
Objective:The study aims to evaluate clinical efficacy of joint-activating ointment in conjunction with arthroscopic microfracture in the repair of cartilage damage associated with middle to late-stage knee osteoarthritis(KOA).Methods:A total of 40 patients diagnosed with cold-dampness-blocked middle to late-stage KOA were enrolled and randomly assigned to either the experimental group or the control group.The experimental group received a combination therapy of joint-activating ointment and arthroscopic microfracture,whereas the control group were treated solely with arthroscopic microfracture.Clinical outcomes were assessed by comparing visual analogue scale(VAS) scores,knee range of motion(ROM),and western ontario and mcmaster universities osteoarthritis index (WOMAC) scores before treatment and at six months post-surgery.Additionally,knee joint space height was measured from anteroposterior radiographs,and cartilage thickness was evaluated using magnetic resonance imaging (MRI).The therapeutic effects on cartilage repair between the two groups were analyzed.Results:Intra-group comparisons revealed that both the VAS and WOMAC scores significantly decreased post-treatment compared to pre-treatment in both groups (all P＜0.01).Furthermore,an increase in knee ROM was observed in both groups after treatment,with statistical significance (all P＜0.05).Between-group comparisons post-treatment demonstrated significant differences in VAS,WOMAC,and ROM (all P＜0.05).The post-treatment values for knee joint space and cartilage thickness were significantly increased compared to pre-treatment values,with statistical significance (all P＜0.01).The experimental group exhibited superior outcomes compared to the control group,with statistically significant differences(all P＜0.05).Conclusion:The combination of joint-activating ointment and arthroscopic microfracture demonstrates favorable clinical efficacy in repairing cartilage damage in middle to late-stage knee osteoarthritis,outperforming the effects of arthroscopic microfracture alone.
