摘要:
目的：探究靳三针疗法联合鼓室注射地塞米松治疗全聋型突发性感音神经性聋（SSNHL）的临床疗效。方法：采用回顾性队列研究设计，纳入70例全聋型SSNHL患者（听力损失≥81 dB HL），分为观察组（35例）和对照组（35例）。观察组采用靳三针（取穴耳三针、颞三针、脑三针）联合鼓室注射地塞米松（5 mg/ml）治疗，对照组仅接受鼓室注射治疗。比较两组患者听力恢复情况、伴随症状改善程度及总体疗效。结果：观察组500、1000、2000 Hz平均听阈改善值显著优于对照组（P<0.001）；观察组眩晕缓解率（72.22% 与 35.29%）和耳鸣改善率（70.37% 与40.86%）均显著高于对照组（均P<0.05），耳闷症状改善两组比较无统计学差异（P>0.05）；观察组总有效率为80.00%，显著高于对照组的57.10%（P<0.05）。结论：靳三针联合鼓室注射地塞米松可通过改善内耳微循环、增强药物靶向性及神经保护等协同作用，显著提高全聋型SSNHL患者的听力恢复率和症状改善效果。该中西医结合方案为治疗难治性突发性聋提供了新思路，其具体机制有待进一步研究证实。
Abstract:
Objective:To investigate the clinical efficacy of Jin’s three-needle therapy combined with intratympanic dexamethasone injection (ITDI) in treating total deafness-type sudden sensorineural hearing loss (SSNHL).Methods:A retrospective cohort study was conducted on 70 patients with total deafness-type SSNHL (hearing loss ≥81 dB HL) were divided into an observation group (n=35,treated with Jin’s three-needle therapy targeting Ear-three-needles,Temporal-three-needles,Brain-three-needles acupoints combined with ITDI (5 mg/ml dexamethasone) and a control group (n=35,ITDI alone).Outcomes included hearing recovery,symptom improvement (vertigo,tinnitus,aural fullness),and overall efficacy.Results:The observation group showed significantly better mean threshold recovery at 500,1000,and 2000 Hz compared to the control group (P<0.001).Vertigo resolution (72.22% vs.35.29%) and tinnitus improvement (70.37% vs.40.86%) were significantly higher in the observation group (P<0.05),while aural fullness improvement showed no intergroup difference (P>0.05).The total effective rate was 80.00% in the observation group versus 57.10% in the control group (P<0.05).Conclusion:The combination of Jin’s Three-Needle therapy and ITDI synergistically enhances hearing recovery and symptom relief in total deafness-type SSNHL,potentially through improved inner ear microcirculation,targeted drug delivery,and neuroprotection.This integrated traditional Chinese and Western medicine approach offers a novel strategy for refractory SSNHL,though its precise mechanisms require further validation.
