摘要:
目的：探究调脏舒痢方直肠滴入联合美沙拉嗪肠溶片治疗溃疡性结肠炎的临床疗效。方法：本研究纳入150例符合大肠湿热型溃疡性结肠炎的患者进行研究，以随机数字表法分为单用药组和联合用药组，各75例。单用药组患者仅接受口服美沙拉嗪肠溶片治疗，而联合用药组患者则在单用药组基础上，通过直肠滴注调脏舒痢方进行辅助治疗。进而对比两组的临床疗效，治疗前后中医症候评分、肠道菌群数量、血清炎症因子[肿瘤坏死因子-α(TNF-α)、白介素(IL)-10、IL-6、IL-1β]水平及不良反应。结果：联合用药组总有效率显著优于单用药组（P<0.05）；治疗后联合用药组各症候评分均显著下降且低于单用药组（P＜0.05）；联合用药组双歧杆菌、乳杆菌数量高于单用药组，大肠杆菌、肠球菌数量低于单用药组；联合用药组TNF-α、IL-1β及IL-6均下降且显著低于单用药组，IL-10水平则升高且显著高于单用药组（P＜0.05）；两组不良反应率对比差异无统计学意义（P＞0.05）。结论：调脏舒痢方直肠滴入联合美沙拉嗪肠溶片治疗溃疡性结肠炎能够有效改善疗效，缓解患者症状，降低炎症反应。
Abstract:
Objective:To investigate the clinical efficacy of rectal administration of Tiaozang Shuli formula combined with oral mesalazine enteric-coated tablets in the treatment of ulcerative colitis (UC).Methods:A total of 150 patients diagnosed with the damp-heat syndrome type of ulcerative colitis were enrolled and randomly divided into two groups (n=75 each) using a random number table method.The monotherapy group received mesalazine enteric-coated tablets orally,while the combination group was treated with additional rectal drip of Tiaozang Shuli formula on the basis of the monotherapy group.Clinical efficacy,Traditional Chinese Medicine (TCM) syndrome scores,intestinal flora counts,serum inflammatory cytokines [tumor necrosis factor-alpha (TNF-α),interleukin (IL)-10,IL-6,IL-1β],and adverse reactions were compared between the two groups before and after treatment.Results:The total effective rate in the combination group was significantly higher than that in the monotherapy group (P<0.05).After treatment,the syndrome scores in the combination group showed a significant reduction and were markedly lower than those in the monotherapy group (P<0.05).The combination group showed significantly higher levels of Bifidobacterium and Lactobacillus,and significantly lower levels of Escherichia coli and Enterococcus compared with the monotherapy group.Additionally,serum levels of TNF-α,IL-1β,and IL-6 were significantly decreased,and IL-10 was significantly increased in the combination group compared to the monotherapy group (P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion:Rectal administration of Tiaozang Shuli formula combined with mesalazine enteric-coated tablets can effectively enhance clinical efficacy,relieve symptoms,and reduce inflammatory responses in patients with ulcerative colitis.
