摘要:
再生障碍性贫血（AA）以骨髓造血衰竭为特征，临床表现为严重贫血、反复出血和反复感染，是一种难治性血液病。该篇介绍了长安张氏肝病流派基于“补肝体强肝用”的思想，提出从肝论治AA，认为肝主疏泄亦可化血调血藏血，其功能失调可影响气血生成与运行，进而累及骨髓造血功能。AA的核心病机在于肝体失养与肝用不达，因而在AA的辨治中，长安张氏肝病流派强调通过滋养肝体以柔肝养血，疏达肝用以调畅气机，对气血两虚、肝脾亏虚、气滞血瘀、肝火旺盛等证型进行辨证论治，形成综合治疗方案，在临床中取得了较好的疗效。本篇通过对临床诊疗思路的总结以及1例临床医案举隅，探讨辨治AA的经验，为临床实践提供新思路。
Abstract:
Aplastic Anemia (AA) is characterized by bone marrow hematopoietic failure,and is clinically manifested as severe anemia,recurrent bleeding and repeated infection,which is a refractory hematologic disease.This article introduces the Chang’an ZHANG’s liver disease school based on the idea of “Nourishing the liver’s substance and strengthening the liver’s function”,and proposes to treat AA from the liver,which believes that the liver can also transform blood and regulate blood and store blood.Its dysfunction can affect the production and operation of qi and blood,and thus affect the bone marrow hematopoietic function.The core pathogenesis of AA lies in the infertility of the liver and the inability to use the liver.Therefore,in the identification and treatment of AA,the Chang’an ZHANG’s liver disease school emphasizes that by nourishing the liver body,softening the liver and nourishing the blood,and relieving the liver to regulate the qi and mechanism,and conducting syndromes such as deficiency of qi and blood,deficiency of liver and spleen,stagnation of qi and blood stasis,and excessive liver fire,forming a comprehensive treatment plan,which has achieved good results in clinical practice.This paper summarizes the clinical diagnosis and treatment approaches and presents a single illustrative case studies to explore the experience in differentiating and treating AA,thereby providing new perspectives for clinical practice.
