摘要:
卒中后抑郁是指在脑血管病基础上发生的情感障碍性疾病，其不仅预后较差，且死亡率远高于无抑郁脑卒中患者。越来越多的证据表明，大脑与胃肠道之间通过“脑-肠轴”进行双向关联，卒中后抑郁与肠道微生物群失调、肠道屏障功能障碍和肠道微生物群代谢物密切相关。通过文献整理分析可知，中医药疗法可通过影响“脑-肠轴”，改变肠道菌群及代谢产物、调节神经递质释放、调控下丘脑-垂体肾上腺轴、减轻免疫炎症反应等机制干预脑肠活动，从而改善卒中后抑郁临床症状。本文从“脑-肠轴”理论角度对中医药治疗卒中后抑郁进行总结，以期为卒中后抑郁的治疗提供中医药理论依据。
Abstract:
Post-stroke depression refers to an affective disorder that occurs on the basis of cerebrovascular disease.It not only has a poor prognosis,but also has a much higher mortality rate than stroke patients without depression.There is increasing evidence that there is a two-way association between the brain and the gastrointestinal tract through the “brain-gut axis”.Post-stroke depression is closely related to intestinal microbiota dysbiosis,intestinal barrier dysfunction and intestinal microbiota metabolites.Through literature analysis,it can be seen that traditional Chinese medicine therapy can interfere with brain-gut activity by affecting the “brain-gut axis”,changing intestinal flora and metabolites,regulating neurotransmitter release,regulating the hypothalamus-pituitary adrenal axis,and reducing immune inflammatory response,thereby improving the clinical symptoms of post-stroke depression.This article summarizes the treatment of post-stroke depression with traditional Chinese medicine from the perspective of “brain-gut axis” theory,in order to provide a theoretical basis for the treatment of post-stroke depression with traditional Chinese medicine.
