摘要:
目的:观察“三阳”透刺劳宫穴联合赤凤迎源手法治疗中风后手指拘挛的临床疗效。方法:将符合纳入标准的70例中风后手指拘挛患者,采用随机数字表法分为观察组和对照组,每组35例。对照组在接受常规内科治疗和康复训练的基础上,采用常规针刺手三阳经输穴。观察组在对照组基础上,于手三阳经输穴向劳宫穴方向进行透刺,并施以赤凤迎源复式补泻手法。两组均每日治疗1次,每周治疗6次,连续治疗3周。分别于治疗前、治疗后评估改良Ashworth量表(MAS)、Fugl-Meyer上肢运动功能评定量表(FMA)、Barthel指数(BI)的评分情况并测量患侧正中神经肌电F波时限变化和手指伸展距离,以及表面肌电图中指浅屈肌、指浅屈肌RMS的客观数据情况,并综合计算临床总体有效率。结果:治疗后,两组患者手指MAS分级均较治疗前改善(P<0.05),且观察组改善程度优于对照组(P<0.05)。两组患者FMA手部评分、BI评分均较治疗前提高,且观察组评分高于对照组(均P<0.05)。治疗后,两组患者的F波时限均较治疗前缩短、手指伸展距离较治疗前增加,且观察组的F波时限缩短及手指伸展距离增加程度均较对照组更为明显(均P<0.05)。与治疗前比较,治疗后两组指浅屈肌、指伸肌RMS均较前降低,且观察组RMS降低程度较为明显(均P<0.05)。观察组临床总有效率为87.50%,高于对照组的65.63%,差异有统计学意义(P<0.05)。结论:“三阳”透刺劳宫穴联合赤凤迎源手法能有效缓解中风后手指拘挛,降低肌张力,并促进手部运动功能和日常生活活动能力的恢复,其疗效优于常规针刺疗法,为临床治疗中风后手指拘挛提供新思路。
Abstract:
Objective:Clinical efficacy of Sanyang penetrating needling at Laogong point combined with Chifeng Yingyuan manipulation in treating finger contracture after stroke was observed.Methods:70 patients with finger contracture after stroke who met inclusion criteria were randomly divided into observation group and control group using random number table,with 35 cases in each group.The control group received conventional internal medicine treatment and rehabilitation training,and were treated with conventional needling at Sanyang transport points.The observation group,on the basis of the control group,received penetrating needling from Sanyang transport points towards Laogong point and was treated with the Chifeng Yingyuan compound reinforcing and reducing manipulation.Both groups were treated once a day,six times a week,for three consecutive weeks.The Modified Ashworth Scale (MAS),Fugl-Meyer Assessment of Upper Extremity (FMA),Barthel Index (BI) scores,the changes in the F-wave latency of the median nerve on the affected side,the finger extension distance,and the objective data of the surface electromyography of the flexor digitorum superficialis and extensor digitorum muscles were evaluated before and after treatment.The overall clinical effective rate was also calculated.Results:After treatment,the MAS grades of both groups were improved compared with those before treatment (P<0.05),and the improvement in the observation group was better than that in the control group (P<0.05).The FMA hand scores and BI scores of both groups were increased compared with those before treatment,and the scores in the observation group were higher than those in the control group (all P<0.05).After treatment,the F-wave latency of both groups were shortened and the finger extension distance were increased compared with those before treatment.The improvement in the observation group were more significant than that in the control group (all P<0.05).Compared with before treatment,the RMS of the flexor digitorum superficialis and extensor digitorum muscles in both groups were decreased after treatment,and the decrease in the observation group was more significant (all P<0.05).The overall clinical effective rate of the observation group was 87.50%,which was higher than 65.63% in the control group,difference statistically significant (P<0.05).Conclusion:Sanyang penetrating needling at the Laogong point combined with the Chifeng Yingyuan manipulation can effectively relieve finger contracture after stroke,reduce muscle tone,and promote the recovery of hand motor function and activities of daily living.Its efficacy was superior to that of conventional needling therapy,providing new approach for clinical treatment of finger contracture after stroke.
