摘要:
目的：应用国家级名老中医周德安“治病先治神”理论，对存在抑郁状态的终末期肾脏病行血液透析患者进行针刺治疗，探讨对血液透析患者抑郁状态、睡眠质量、血透相关营养指标、血液透析生活质量以及并发症的影响。方法：选取60例血液透析过程中被诊断为抑郁的患者，分为治疗组与对照组，各30例。治疗组应用周德安针灸四神方加开四关进行针刺治疗，对照组采用非穴浅刺，两组治疗周期均为8周，比较治疗前、治疗后8周、治疗后16周抑郁状态、睡眠质量、血透相关营养指标及生活质量，并比较两组不良事件发生率。结果：治疗后8周、16周两组MADRS、BDI-Ⅱ评分均低于治疗前，治疗组治疗后8周、16周MADRS评分、BDI-Ⅱ评分均低于对照组（P＜0.05）；治疗组治疗后8周、16周ISI、PSQI降低，对照组无明显变化，治疗组16周ISI、PSQI均高于对照组（P＜0.05）；治疗组治疗后8周、16周血清白蛋白（ALB）、体重指数（BMI）水平升高，对照组无明显变化，治疗组治疗后8周、16周ALB、BMI均高于对照组（P＜0.05）；治疗后8周、16周两组生理健康（PCS）和心理健康（MCS）均升高，且均高于对照组（P＜0.05）；两组不良事件发生率比较无统计学差异（P＞0.05）。结论：基于周德安“治病先治神”理论针灸治疗对血液透析合并抑郁状态效果显著，可改善患者抑郁状态，辅助提升睡眠质量，改善患者相关营养指标，提升生活质量，安全性较高。
Abstract:
Objective:To apply the theory of “treating the spirit before treating the disease” by national famous old TCM doctor ZHOU De’an,acupuncture was used to treat patients with end-stage renal disease and hemodialysis who were depressed,and the effects on depression,sleep quality,hemodialysis-related nutritional indicators,quality of life and complications were explored.Methods:A total of 60 patients diagnosed with depression during hemodialysis we redivided into a treatment group (30 patients) and a control group (30 patients).The treatment group received acupuncture therapy using ZHOU De’an’s Four Gods Formula combined with Kai Siguan acupoints,while the control group underwent non-acupoint superficial needling.Both groups underwent 8-week treatment cycles.The study compared depression status,sleep quality,hemodialysis-related nutritional indicators,and quality of life before treatment,at 8 weeks post-treatment,and 16 weeks post-treatment,while also evaluating adverse event rates between the two groups.Results:At 8 and 16 weeks after treatment,both groups showed lower MADRS and BDI-Ⅱ scores compared to before treatment.The treatment group had lower MADRS and BDI-Ⅱ scores than the control group at 8 and 16 weeks after treatment (P<0.05).At 8 and 16 weeks after treatment,the treatment group’s ISI and PSQI scores decreased,while the control group showed no significant change;the treatment group’s ISI and PSQI scores at 16 weeks were higher than those of the control group (P<0.05).At 8 and 16 weeks after treatment,the treatment group showed increased serum albumin (ALB) and body mass index (BMI) levels,whereas the control group showed no significant change;ALB and BMI in the treatment group at 8 and 16 weeks were higher than in the control group (P<0.05).At 8 and 16 weeks after treatment,both groups showed improvements in physical health (PCS) and mental health (MCS),with the treatment group were higher than those of the control group (P<0.05).There was no significant difference in the incidence of adverse events between the treatment and control groups (P>0.05).Conclusion:Acupuncture and moxibustion treatment based on ZHOU De’an’s theory of “treating the disease first and treating the spirit” has a significant effect on hemodialysis patients with depression,which can improve the depressive state of patients,assist in improving sleep quality,improve patients’ related nutritional indicators,and improve their quality of life with high safety.
