摘要:
史纪教授纵览温病学医籍,通过病机溯源,提出肺炎支原体属于温邪范畴。基于“温邪则热变”“温毒-肺络”理论归纳出儿童重症肺炎支原体肺炎(SMPP)核心病机为温毒伤络、痰瘀互结,依据“察舌验苔”洞悉“扭转截断点”,并据此分为苗期、极期、恢复期,主张苗期疏散先行,辅以解毒通络,极期解毒为主,重以活血通络,恢复期解余邪,佐以养阴通络,以苇茎清肺汤为基础方,结合“性味合化”理论,分期加减论治。史纪教授从毒损肺络辨治儿童SMPP,理论依据充实可靠,临证具有较好的临床疗效,为儿童SMPP诊治提供理论依据和临床借鉴。
Abstract:
After extensively reviewing the medical literature on warm diseases and tracing their pathogenesis,SHI Ji proposed that Mycoplasma pneumoniae (MP) falls under the category of warm pathogens.Based on the theory of “warm pathogens leading to heat transformation” and “warm toxin-lung collaterals”,he summarized the core pathogenesis of severe Mycoplasma pneumoniae pneumonia (SMPP) in children as warm toxin damaging collaterals and phlegm-blood stasis intermingling.By observing the tongue and coating,he identified the “critical interception point” and divided the disease into three stages:initial,peak,and recovery.He advocated for early dispersion in the initial stage,supplemented by detoxification and collateral unblocking.Prioritized detoxification in the peak stage,with emphasis on activating blood circulation and unblocking collaterals.And focused on resolving residual pathogens in the recovery stage,assisted by nourishing Yin and unblocking collaterals.Using Weijing Qingfei decoction as the foundational prescription.By combining the “theory of flavor and property compatibility”,he tailored treatments according to disease stages.Professor SHI Ji’s approach to the diagnosis and treatment of pediatric SMPP from the perspective of toxin damage to lung collaterals is theoretically robust and clinically effective,providing a theoretical basis and clinical reference for SMPP management in children.
