摘要:
目的:探讨等速离心训练联合电针治疗对肩峰下撞击综合征患者肩关节功能及生物力学指标的改善作用。方法:选取符合标准的肩峰下撞击综合征者110例,采用随机数字表法分组。随机序列由独立统计师生成,并使用密封信封进行分配隐藏,由不参与分组的独立管理员在患者签署知情同意书后立即实施分配。将患者分为对照组(55例)和治疗组(55例)。对照组接受条口穴电针和肩部主动运动训练,治疗组在此基础上增加等速离心训练,共干预8周。治疗前后采用Constant-Murley肩关节功能评分(CMS)、视觉模拟评分法(VAS)及等速肌力测试(功能性比率FR、加速时间AT、达峰力矩时间TPT)进行疗效评估。结果:治疗后,治疗组CMS总分显著高于对照组(P<0.05)。在等速肌力指标方面,治疗组在60°/s和120°/s角速度下的FR值均优于对照组(P<0.05),AT和TPT也较对照组显著缩短(P<0.05)。治疗组AT和TPT较对照组显著缩短,两组VAS评分均较治疗前显著降低(P<0.05),但组间比较无统计学意义(P>0.05)。结论:IET联合电针可改善SAIS患者肩关节肌力平衡和神经肌肉反应速度,能间接增强动态稳定性,促进功能恢复。IET优化肩袖离心控制以增强力学平衡,电针通过调节炎症与神经肌肉通路缓解疼痛并促进修复。两者的“力学–神经-免疫”协同作用优于单一干预,为中西医结合康复提供机制依据。
Abstract:
Objective:To explore improvement effect of isokinetic eccentric training combined with electroacupuncture on shoulder joint function and biomechanical indicators in patients with subacromial impingement syndrome.Methods:A total of 110 patients with subacromial impingement syndrome who met criteria were selected and randomly divided into groups using random number table.The random sequence was generated by an independent statistician and allocated using sealed envelopes.An independent administrator who was not involved in grouping immediately implemented the allocation after patients signed the informed consent form.Patients were divided into control group (55 cases) and treatment group (55 cases).The control group received electroacupuncture at Tiaokou point and active shoulder movement training,while the treatment group received isokinetic eccentric training in addition to the above.The intervention lasted for 8 weeks.The constant-murley shoulder function score(CMS),visual analogue scale (VAS),and isokinetic muscle strength test (functional ratio FR,acceleration time AT,time to peak torque TPT) were used to evaluate the therapeutic effect before and after treatment.Results:After treatment,the total CMS score of the treatment group was significantly higher than that in the control group (P<0.05).In terms of isokinetic muscle strength indicators,FR values of the treatment group at 60°/s and 120°/s angular velocities were significantly better than those in the control group (P<0.05),and the AT and TPT were also significantly shorter than those in the control group (P<0.05).The AT and TPT of the treatment group were significantly shorter than those in the control group,and the VAS scores of both groups were significantly lower than those before treatment (P<0.05).There was no statistical difference between the two groups (P>0.05).Conclusion:IET combined with electroacupuncture can improve the muscle strength balance and neuromuscular response speed of the shoulder joint in patients with subacromial impingement syndrome,indirectly enhance dynamic stability,and promote functional recovery.IET optimizes the eccentric control of the rotator cuff to enhance mechanical balance,while electroacupuncture alleviates pain and promotes repair by regulating inflammation and neuromuscular pathways.Mechanical-neuro-immune synergistic effect of the two was superior to single intervention,providing mechanism basis for the combination of traditional Chinese and Western medicine in rehabilitation.
