摘要:
目的：观察补气养血通乳方熏蒸联合子午流注开穴乳房低频脉冲电刺激治疗产后缺乳的临床效果。方法：选取94例产后缺乳患者，采用随机法分为两组，各47例。两组均接受母乳喂养指导、乳房局部按摩等，对照组给予子午流注开穴乳房低频脉冲电刺激治疗，观察组在对照组基础上联合补气养血通乳方熏蒸治疗。比较两组总有效率、泌乳素（PRL）、孕酮（P）、雌二醇（E2）、气血虚弱证评分、配方奶粉补充量、乳汁情况以及不良反应发生率。结果：观察组总有效率93.62%高于对照组的78.72%，差异有统计学意义（P＜0.05）。治疗前两组P、E2、PRL比较差异无统计学意义（P>0.05）；治疗后两组P、E2均较治疗前下降，观察组更低（P<0.05）；两组治疗后PRL均较治疗前升高，观察组更高（P<0.05）。两组治疗前气血虚弱证评分比较差异无统计学意义（P>0.05）；两组治疗后心悸气短、面色少华、神疲食少以及手足发麻评分均较治疗前下降，观察组更低（P<0.05）。两组治疗前配方奶粉补充量比较发现差异无统计学意义（P>0.05）；两组治疗后配方奶粉补充量均较治疗前下降，观察组更低（P<0.05）。两组治疗前乳汁情况比较差异无统计学意义（P>0.05）；治疗后两组乳汁量、乳汁能量、乳汁密度均较治疗前升高，观察组更高（P<0.05）。观察组总不良反应发生率6.38%与对照组的2.13%比较，差异无统计学意义（P>0.05）。结论：产后缺乳患者采用补气养血通乳方熏蒸联合子午流注开穴乳房低频脉冲电刺激可提高乳汁能量和密度，促进泌乳量，减少婴儿配方奶粉补充量。
Abstract:
Objective:To observe the clinical effect of the combination of the Buqi Yangxue Tongru formula fumigation and the low-frequency pulse electrical stimulation of the breast based on the theory of Ziwu Liuzhu and opening acupoints in the treatment of postpartum hypogalactia.Methods:A total of 94 patients with postpartum lactation insufficiency admitted were randomly divided into two groups of 47 patients each.Both groups received breastfeeding guidance and local breast massage.The control group received meridian-based low-frequency pulse electrotherapy for breast stimulation,while the observation group additionally underwent steam therapy with a Buqi Yangxue Tongru formula.The following parameters were compared between groups:overall response rate,prolactin (PRL),progesterone (P),estradiol (E2),Qi and Blood Deficiency Syndrome scores,formula supplementation volume,lactation status,and adverse reaction incidence.Results:The total effective rate in the observation group 93.62%was higher than that in the control group 78.72%,with a statistically significant difference (P<0.05).Comparison of pre-treatment levels of P,E2,and PRL between the two groups showed no statistically significant difference (P>0.05);After treatment,both groups showed decreased levels of P and E2 compared to pre-treatment levels,with post-hoc analysis indicating lower levels in the observation group (P<0.05).Both groups exhibited increased PRL levels post-treatment compared to pre-treatment,with post-hoc analysis showing higher levels in the observation group (P<0.05).Pre-treatment Qi and Blood Deficiency Syndrome scores showed no statistically significant difference between groups (P>0.05);After treatment,scores for palpitations and shortness of breath,pale complexion,fatigue and poor appetite,and numbness in hands and feet decreased in both groups compared to pre-treatment levels,with post-hoc analysis indicating lower scores in the observation group (P<0.05).Comparison of pre-treatment formula milk supplementation levels between groups showed no statistically significant difference (P>0.05);After treatment,both groups showed reduced formula milk supplementation compared to pre-treatment levels,with the observation group exhibiting a significantly lower amount (P<0.05).Pre-treatment breast milk status showed no statistically significant difference between groups (P>0.05).Post-treatment breast milk volume,energy content,and density increased in both groups compared to pre-treatment levels,with the observation group demonstrating significantly higher values (P<0.05).The incidence of adverse reactions in the observation group 6.38% was not statistically significantly different from that in the control group 2.13% (P>0.05).Conclusion:The combination of the Buqi Yangxue Tongru formula fumigation and the low-frequency pulse electrical stimulation of the breast based on the theory of Ziwu Liuzhu and opening acupoints can promote lactation,increase the energy and density of breast milk,and reduce the supplementation of infant formula milk.
