摘要:
儿童呼吸系统炎症(如哮喘、急性支气管炎、肺炎等)是儿科临床高发疾病,其发病机制与炎症信号通路异常激活密切相关。NF-κB/STAT3信号通路作为调控炎症反应的核心通路,通过介导促炎因子(TNF-α、IL-6、IL-1β等)的释放和炎症细胞浸润,在儿童呼吸系统炎症的发生、发展中发挥关键作用。芳香中药挥发油作为中药的重要活性成分,具有挥发性强、生物利用度高、抗炎活性显著且不良反应小等特点,契合儿童用药“安全、高效、低毒”的需求。本文系统综述了儿童呼吸系统炎症的病理特征及NF-κB/STAT3信号通路的调控作用,总结了常见芳香中药挥发油(薄荷油、薰衣草油、紫苏叶油等)的抗炎活性及分类特征,重点阐述其通过抑制NF-κB核转录、下调STAT3磷酸化、阻断通路交叉对话、调控表观遗传修饰及与通路蛋白分子对接等分子机制调控儿童呼吸系统炎症的研究进展,分析当前研究中存在的剂型稳定性、给药途径优化、儿童用药安全性评价等问题,并对未来剂型创新、多通路协同机制研究及临床转化应用进行展望,为芳香中药挥发油在儿童呼吸系统炎症治疗中的进一步开发和临床应用提供参考。
Abstract:
Childhood respiratory inflammation (e.g.,asthma,acute bronchitis,pneumonia) is a clinically prevalent disease in pediatrics,whose pathogenesis is closely associated with the abnormal activation of inflammatory signaling pathways.As the core pathway regulating inflammatory responses,the NF-κB/STAT3 signaling pathway plays a pivotal role in the occurrence and progression of childhood respiratory inflammation by mediating the release of pro-inflammatory factors (TNF-α,IL-6,IL-1β,etc.) and the infiltration of inflammatory cells.Volatile oils from aromatic traditional Chinese medicine (TCM),as important active components of TCM,exhibit characteristics including high volatility,good bioavailability,significant anti-inflammatory activity,and low toxicity,which meet the requirements for pediatric medications of “safety,efficacy,and low toxicity”.This review systematically summarizes the pathological characteristics of childhood respiratory inflammation and the regulatory role of the NF-κB/STAT3 signaling pathway,outlines the anti-inflammatory activity and classification features of common volatile oils from aromatic TCM (e.g.,peppermint oil,lavender oil,perilla leaf oil),and focuses on elaborating the research progress of their regulatory effects on childhood respiratory inflammation through molecular mechanisms such as inhibiting NF-κB nuclear transcription,downregulating STAT3 phosphorylation,blocking pathway crosstalk,regulating epigenetic modifications,and molecular docking with pathway proteins.Additionally,it analyzes the existing problems in current research,including dosage form stability,optimization of administration routes,and safety evaluation of pediatric medications,and provides prospects for future dosage form innovation,research on multi-pathway synergistic mechanisms,and clinical transformation and application.This review aims to provide a reference for the further development and clinical application of volatile oils from aromatic TCM in the treatment of childhood respiratory inflammation.
