摘 要 目的:探讨丹参多酚酸盐在急诊缺血性心肌损伤患者治疗中的应用效果。方法:研究ST段抬高型心肌梗死(STEMI)患者104例,以随机数表法分为对照组与研究组各52例。对照组根据《急性心力衰竭诊断和治疗指南》采取规范药物治疗;在此基础上,研究组应用丹参多酚酸盐进行治疗。评价两组治疗前与治疗14 d时血清丙二醛(MDA)与超氧化物歧化酶(SOD)、心肌酶学指标\[肌酸激酶同工酶(CK-MB)、肌酸激酶(CK)、乳酸脱氢酶(LDH)\]、血液流变学指标(血浆比黏度、全血比黏度、血小板黏附率与红细胞压积)的变化,并统计治疗期间的药物不良反应。结果:治疗后研究组MDA与SOD低于对照组(P<0.05)。治疗后研究组CK-MB、CK与LDH水平低于对照组(P<0.05)。治疗后研究组血浆比黏度、全血比黏度、血小板黏附率与红细胞压积低于对照组(P<0.05)。治疗后研究组血乳酸指标低于对照组(P<0.05)。对照组与研究组治疗期间各出现头晕1例(1.92%),不良反应发生率对比差异无统计学意义(P>0.05)。结论:在规范用药治疗的基础上对缺血性心肌损伤患者使用丹参多酚酸盐,能够有效改善心肌酶学与血液流变学指标,抑制心肌损伤,安全可靠 。
Abstract:
Abstract Objective:To investigate the effect of salvianolate in patients with acute ischemic myocardial injury.Methods:104 patients with ST-segment elevation myocardial infarction(STEMI) were divided into control group and research group by random number table method,52 cases in each group.The control group was treated with standard drugs according to the guidelines for diagnosis and treatment of acute heart failure;on this basis,the study group was treated with salvianolate.The changes of Serum malondialdehyde(MDA),Superoxide dismutase(SOD),myocardial enzymatic indexes \[Creatine kinase isoenzyme(CK-MB),Creatine kinase(CK),Lactate dehydrogenase(LDH)\],hemorheological indexes(plasma specific viscosity,whole blood specific viscosity,platelet adhesion rate and hematocrit) were evaluated before and 14 days after treatment in the two groups,and adverse drug reactions during treatment were counted.Results:After treatment,MDA and SOD in the study group were lower than those in the control group(P<0.05).After treatment,CK-MB,CK and LDH in the study group were lower than those in the control group(P<0.05).After treatment,the plasma specific viscosity,whole blood specific viscosity,platelet adhesion rate and hematocrit in the study group were lower than those in the control group(P<0.05).After treatment,the blood lactic acid index in the study group was lower than that in the control group(P<0.05).During the treatment period,dizziness occurred in 1 case 1.92%in the control group and the control group.There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Salvia miltiorrhiza polyphenolic salts can effectively improve myocardial enzymology and hemorheology indexes,inhibit myocardial injury,and is safe and reliable for patients with ischemic myocardial injury.
