摘 要 目的：探讨血府逐瘀汤联合西药治疗肺间质纤维化患者的临床应用效果。方法：选择肺间质纤维化患者120例为研究对象，按随机分配原则分为观察组和对照组，各60例。对照组给予常规西药治疗，观察组在对照组基础上联合血府逐瘀汤内服，方剂组成为桃仁、红花、当归、生地黄、牛膝、川芎、桔梗、赤芍、枳壳、甘草。对两组患者的临床疗效、肺功能指标、血清炎症因子及用药安全性进行综合评价对比。结果：观察组临床总有效率为88.33%(53/60)，高于对照组73.33%(44/60)，组间差异比较有统计学意义(χ2=4.357,P=0.037)。两组患者治疗4周后FEV1、FVC、PEF均较治疗前提高，且观察组高于对照组(t=4.389、6.348、7.198，P＜0.01)。两组患者治疗4周后CRP、TNF-α、IL-1β均较治疗前降低，观察组低于对照组(t=5.894、8.125、8.613，P＜0.01)。两组患者治疗期间不良反应发生率比较差异无统计学意义(χ2=1.046,P=0.306)。结论：血府逐瘀汤辅助西药治疗肺间质纤维化，临床疗效较好，可有效改善患者肺功能指标，降低患者体内炎症因子水平，且不增加不良反应发生率。
Abstract:
Abstract Objective：To study the clinical effect of Xuefu Zhuyu decoction in the treatment of pulmonary interstitial fibrosis.Methods：120 patients with pulmonary interstitial fibrosis were selected as the research objects.They were divided into observation group and control group according to the principle of random distribution，60 cases in each group.The control group was given conventional western medicine，and the observation group was combined with Xuefu Zhuyu decoction.The components of the prescription were Taoren，Honghua，Danggui，Shengdihuang，Niuxi，Chuanxiong，Jiegeng，Chishao，Zhiqiao，Gancao.The clinical efficacy，pulmonary function index，serum inflammatory factors and medication safety of the two groups were compared comprehensively.Results：The total clinical effective rate of the observation group was 88.33%(53/60),which was higher than 73.33%(44/60) of the control group,the difference between the two groups was statistically significant(χ2=4.357,P=0.037).FEV1，FVC and PEF of the two groups after 4 weeks of treatment were all higher than those before treatment,and the observation group was higher than that of the control group(t=4.389、6.348、7.198，P＜0.01).After 4 weeks of treatment，the levels of CRP，TNF-α and IL-1β in the two groups were lower than those before treatment，and those in the observation group were lower than those in the control group(t=5.894、8.125、8.613，P＜0.01).There was no significant difference in the incidence of adverse reactions between the two groups during treatment(χ2=1.046,P=0.306).Conclusion：Xuefu Zhuyu decoction combined with western medicine has good clinical effect in treating pulmonary interstitial fibrosis.It can improve the pulmonary function index of patients，reduce the level of inflammatory factors in patients，and has good therapeutic safety，which is worthy of clinical recommendation.
