摘要:
目的:分析总结儿童过敏性紫癜不同中医证型在临床特征上的分布规律并探讨儿童过敏性紫癜的中医治疗方法。方法:收集并整理171例过敏性紫癜(HSP)患儿的临床资料,对比中医证型在不同性别、年龄、病程、临床症状、诱因中的分布情况; 总结171例HSP患儿中药的使用情况并结合相关文献复习。结果:①171例HSP患儿中风热伤络证86例(50.92%),血热妄行证36例(21.05%),湿热痹阻证49例(28.66%),三种证型在男女性别中的分布差异不明显; 在病程上,三种证型均以10～20 d居多,总计98例(57.31%); 在不同年龄段、临床症状中的分布存在统计学差异(P<0.05)。②儿童HSP的常用药可分为解表药、清热药、化湿药、利水渗湿药、止血药、活血化瘀药、补虚药、理气药等。结论:HSP患儿以风热伤络证最为多见,临床病程长短不一,症状多样,不同证型的治疗方法及疗效不同,中医药从不同角度治疗儿童HSP均取得了显著效果。
Abstract:
Objective:Analyze and summarize the distribution of different TCM syndromes in children with allergic purpura in the clinical characteristics and explore the TCM treatment methods for children with allergic purpura.Methods:Collect and sort out the clinical data of 171 children with Henoch-schonlein purpura(HSP),compare the distribution of TCM syndromes in different gender,age,course of disease,clinical symptoms,and inducements; summarize the use of traditional Chinese medicine in 171 children with HSP and review the relevant literature.Results:①Among 171 cases of children with HSP,there are 86 cases(50.92%)of wind-heat damaging collaterals syndrome,36 cases(21.05%)of blood-heat damaging collaterals syndrome and 49 cases(28.66%)of damp-heat damaging collaterals syndrome.The distribution of syndromes among male and female shows no significant difference; in the course of the disease,the three types of syndromes are mostly 10-20 days with a total of 98 cases(57.31%); there are statistical differences in the distribution of clinical symptoms in different age groups(P<0.05).②Medicines commonly used for children's HSP can be divided into anti-surface medicines,heat-clearing medicines,dampness medicines,diuresis and dampness medicines,hemostatic medicines,blood promoting medicines,tonic medicines,etc.Conclusion:The wind-heat damaging collaterals syndrome is mostly found in children with HSP,with different clinical course and diverse symptoms,different treatment methods and curative effects of various syndrome types.Traditional Chinese medicine has achieved remarkable results in treating children's HSP from different angles.
